Knee replacement in chronic post-traumatic cases

Abstract: Post-traumatic knee arthritis is a challenging condition. Prosthetic surgery is demanding and the risk of complications is relatively high.Planning is an essential element of this surgery; correct diagnosis (to exclude latent infection) and adequate considerations regarding approach, axis, bone loss, choice of implant and level of constraint are indispensable. There are two main categories of post-traumatic arthritis: extra-articular deformities and articular deformities.Use of an algorithms can support the sur… Show more

“…The choice of the three aetiological groups (fracture, ligament injury, and osteotomy) may appear criticisable but is fully consistent with the objective of the 2016 SoFCOT symposium. Osteotomy carries the same risks and requires the same precautions as previous trauma to the knee (history of surgery with a previous approach and possible ligament damage) [8][9][10].…”

“…The patients were evaluated during visits before and after the TKA procedure. The Knee injury and Osteoarthritis Outcome Score (KOOS) was determined at last follow-up [10] but not before TKA. The radiographic work-up performed before TKA and at last followup consisted of antero-posterior and lateral weight-bearing views, antero-posterior telemetry to determine the mechanical axis of the lower limb (HKA angle), and a tangential femoro-patellar view with the knee flexed at 60 • .…”

“…The deformity leads to chronic abnormal stress on joint surfaces and subsequent cartilage degeneration. 8 In this study, we aimed to present a patient with PD who had recurrent falls and conservative treatment-resistant monoarthritis secondary to falls. We think that traumatic monoarthritis and recurrent falls in patients with PD might improve by effective surgical treatment and rehabilitation programme when they do not respond to conservative treatment.…”

“… 1) This early degenerative change of the articular cartilage is defined as posttraumatic osteoarthritis (PTOA). 2) Approximately 21%–44% of patients with a history of lower extremity fractures will consequently develop PTOA. 3) …”

“… 4 5) However, TKA in the presence of PTOA is challenging since it usually involves bony nonunion/malunion, intra- or extra-articular deformity, soft-tissue problems, as well as retention of hardware that may hinder accurate restoration of the lower limb alignment and proper implant position. 1 2) …”
