Investigation of the efficacy of collagen membranes used in the full-thickness hyaline cartilage defect surgery is extremely urgent from the point of view of everyday healthcare. However, there is no information about the collagen membrane transformation timeframe, patterns and type of tissue the membrane transforms into, nor on the quality of the newly formed cartilage, which hinders the use of collagen membranes in clinical practice. This study aimed to investigate the biological potential of collagen membranes and their capacity to transform into cartilage tissue. The study involved four pigs as subjects. We induced a full-thickness cartilage defect on their right hind limb joint and implanted an Ortokeep collagen membrane to remedy it. Two full-thickness cartilage defects were induced on the left hind limb joints of the animals, one was treated with an implanted Chondro-Gide collagen membrane, the other remained without a membrane. The animals were withdrawn from the experiment at 2, 3, 4, 6 months after the operation. This report contains results of the macroscopic and microscopic analyses revealing the character of cartilage tissue regeneration at various timepoints post-surgery. The collagen membranes proved to have a high biological potential and a capacity to transform into cartilage tissue. The cartilages were identifiable from the 3rd month of the study. Their thickness was growing significantly (p < 0.05) up to the 4th month post-surgery, gaining 18.7% in group 1 and 12.8% in group 2; afterwards, the formed tissue "matured". We have shown that the AMIC technique allows significant (p < 0.05) reduction of the bone tissue destruction area.

Izuchenie effektivnosti primeneniya kollagenovyh membran pri hirurgicheskom lechenii polnoslojnyh defektov gialinovogo hryashcha krajne aktual'no dlya prakticheskogo zdravoohraneniya. Otsutstvie svedenij o tom, v kakie sroki, kak i v kakuyu hryashchevuyu tkan' transformiruyutsya kollagenovye membrany, kakovo kachestvo vnov' obrazovannogo hryashcha, sderzhivaet ih primenenie v klinicheskoj praktike. Cel'yu issledovaniya bylo izuchit' biologicheskij potencial kollagenovyh membran i ih sposobnost' k transformacii v hryashchevuyu tkan'. Issledovanie provodili na chetyrekh svin'yah. Na sustavah pravyh zadnih konechnostej formirovali polnoslojnyj defekt hryashcha i implantirovali kollagenovuyu membranu Ortokeep. Na sustavah levyh zadnih konechnostej formirovali po dva polnoslojnyh defekta hryashcha. Na odin defekt implantirovali kollagenovuyu membranu ChondroGide, na vtoroj defekt membranu ne implantirovali. ZHivotnyh vyvodili iz eksperimenta v sroki 2, 3, 4, 6 mesyacev posle operacii. Predstavleny makroskopicheskij i mikroskopicheskij analiz haraktera regeneracii hryashchevoj tkani v razlichnye sroki posle operacii. Rezul'taty pokazali vysokij biologicheskij potencial kollagenovyh membran i ih vozmozhnost' transformirovat'sya v hryashchevuyu tkan'. Hryashch vyyavlyalsya s 3-go mesyaca issledovaniya. Otmechena tendenciya k statisticheski dostovernomu (p < 0,05) uvelicheniyu ego tolshchiny vplot' do 4-go mesyaca (v gruppe 1 — na 18,7%, vo gruppe 2 — na 12,8%) s posleduyushchim ego «sozrevaniem». Pokazano, chto pri ispol'zovanii tekhnologii AMIC statisticheski dostoverno (p < 0,05) umen'shaetsya oblast' destrukcii kostnoj tkani.

The article presents an analytical review on modern ideas about the osteoarthritis pathogenesis based on the findings regarding the subchondral bone and its importance in the development of this disease. It is shown that the data of numerous studies in recent years reveal more and more evidence demonstrating the primacy of pathological changes in the subchondral bone in the development of osteoarthritis and its progression. The vast majority of scientific papers confirm the fact that hyaline cartilage and subchondral bone tissue are a single morphofunctional biocomposite with an interdependent system of biochemical connections and molecular signaling, as well as correlative reactions to stressful mechanical loads. The authors analyzed in detail the mechanisms of cellular and molecular interaction in the system "hyaline cartilage subchondral bone" in the development of osteoarthritis, vividly demonstrating the active and priority involvement of subchondral bone tissue in the debut and maintenance of the destructive-dystrophic process. The necessity to leave the chondrocentric model of osteoarthritis pathogenesis and the expediency to revise the points of application of therapeutic measures in patients with knee joint osteoarthritis are discussed. The current methods of surgical treatment of knee joint osteoarthritis are critically reviewed from the perspective of their pathogenetic orientation. The authors discuss the relevance in developing the concept of organ-preserving surgery in destructive-dystrophic joint lesions, which should be based on the findings describing the role and significance of subchondral and metaphyseal bone tissue in the above pathologic processes.
