Zusammenfassung: Gegenstand und Ziel: In der vorliegenden Arbeit wurden histologische Parameter vakzineassoziierter feliner Fibrosarkome in Zusammenhang mit klinischen Verlaufsdaten retrospektiv untersucht. Ziel der Studie war, histologische Beurteilungskriterien zu ermitteln, die eine prognostische Aussage bezüglich der postoperativen rezidivfreien Zeit und der Überlebenszeit der Tiere ermöglichen. Material und Methoden: Das Patientengut umfasst 41 Katzen, die an einer typischen Injektionsstelle ein Fibrosarkom entwickelt hatten. Die Tumoren wurden chirurgisch entfernt und histologisch untersucht. Dabei wurden bei jedem Fibrosarkom zahlreiche histologische Untersuchungskriterien zunächst einzeln beurteilt und mit einem Punktesystem bewertet. Anschließend wurden verschiedene histologische Kriterien kombiniert und mehrere Variationen eines Gesamtscores erstellt. Bei jeder Katze erfolgte eine Dokumentation des weiteren klinischen Verlaufs über einen Beobachtungszeitraum von mindestens einem Jahr. Ergebnisse: 28 von 41 Katzen (68%) entwickelten innerhalb des ersten Jahres post operationem Rezidive, die durchschnittlich nach 145 Tagen (d) auftraten (Median 90 d, Minimum 21 d, Maximum 325 d). Katzen mit Primärtumoren hatten eine signifikant bessere Prognose hinsichtlich der postoperativen rezidivfreien Zeit als Tiere, die bereits bei Aufnahme in die Studie mit einem Rezidiv vorgestellt wurden. Keines der histologischen Beurteilungskriterien stand einzeln berechnet in einem statistisch signifikanten Zusammenhang mit der rezidivfreien Zeit oder der Gesamtüberlebenszeit. Erst nach Bildung eines Gesamtscores aus Kombinationen mehrerer histologischer Kriterien ließ sich ein Zusammenhang mit der Prognose statistisch belegen. Dabei scheint das Ausmaß des infiltrativen Wachstums der Fibrosarkome eine entscheidende Rolle zu besitzen. Schlussfolgerungen: Fibrosarkome rezidivieren häufig. Auch histologisch wenig maligne erscheinende Tumoren können eine schlechte Prognose aufweisen. Klinische Relevanz: Ein Grading vakzineassoziierter feliner Fibrosarkome ist nur sinnvoll, wenn die Infiltrationsneigung der Tumoren mit berücksichtigt wird.

ZusammenfassungBeim Hund unterscheidet man verschiedene histiozytäre Erkrankungen, die durch eine Proliferation histiozytärer Zellen (Makrophagen und myeloisch-dendritische Zellen) entstehen. Differenziert werden neoplastische (kutaner Histiozytomkomplex, histiozytäres Sarkom, dendritische Leukämie) von reaktiven Formen (reaktive Histiozytose, hämophagozytäres Syndrom). Alle Unterformen des kutanen Histiozytomkomplexes (kutanes Histiozytom, metastasierendes Histiozytom und Langerhans-Zell-Histiozytose) gehen von Langerhans-Zellen aus. Das als solitärer Hauttumor bei jungen Hunden auftretende kutane Histiozytom zeigt zumeist eine spontane Regression. Zum Teil kommt es zur Metastasierung in die Lymphknoten (metastasierendes Histiozytom). Die nur einmal beschriebene Langerhans-Zell-Histiozytose erforderte die Euthanasie des betroffenen Hundes. Das von myeloisch-dendritischen Zellen ausgehende histiozytäre Sarkom (HS) wird in eine lokalisierte (LHS) und eine disseminierte Form (DHS) unterteilt. Das hämophagozytäre histiozytäre Sarkom (HHS) geht von Makrophagen aus. Das HS zeigt einen sehr aggressiven klinischen Verlauf und hat eine schlechte Prognose. Für das DHS und das HHS bestehen Rassedispositionen für den Berner Sennenhund, den Rottweiler und verschiedene Retrieverrassen. Die reaktive Histiozytose (kutane, systemische Form) ist eine reaktive Proliferation interstitiell-dendritischer Zellen. Für die systemische Form gelten ähnliche Rassedispositionen wie für das HS. Das hämophagozytäre Syndrom stellt eine Proliferation aktivierter Makrophagen in verschiedenen Organen dar. Die Prognose ist mäßig und richtet sich nach der auslösenden Grunderkrankung.
