The kallikrein-kinin system in the kidney is localized in the distal nephron, where it appears to be linked to processes that control water and electrolyte excretion. Some evidence exists that the effect of the renal kallikrein-kinin system is partially mediated by the release of prostaglandins. It has not yet been determined whether endogenous kinins affect the function of the nephron directly or indirectly by changes in renal blood flow distribution. Further, a number of studies in animals and humans indicates that kallikrein excretion is decreased in most types of hypertension, with the exception of hypertension caused by an excess of mineralocorticoids (where kallikrein is increased). In rats susceptible to the hypertensive effect of salt, kallikrein is conspicuously decreased. In renal diseases, urinary kallikrein excretion is also decreased. Finally, it still needs to be determined whether or not low kallikrein excretion is a pathogenetic factor in hypertension and renal diseases.

“…To date, however, few patients with liver disease have been examined for alterations in the plasma prekallikrein-bradykininogen system. Colman et al (1969a) found reduced plasma prekallikrein levels in six patients with cirrhosis consistent with increased activation of the system whereas Carretero, Nasjletti, Inon, and Dohmen (1970) found no significant changes in the kinin and kininogenase activity of the plasma in five patients with liver failure. The latter authors also found lower plasma prekallikrein in five patients with liver failure, and lower plasma bradykininogen levels which they attributed to impaired synthesis by the damaged liver.…”

The plasma prekallikrein-bradykininogen system was studied in 45 patients with chronic liver disease since its activation with increased liberation of kinin into the plasma could account for some of the clinical manifestations of cirrhosis, namely, vasodilatation, hypotension, and increased capillary permeability.A significant reduction in plasma bradykininogen was found in the cirrhotic patients as compared with control groups of normal subjects and hospital inpatients. The mean plasma prekallikrein was not significantly different and only five patients with liver disease had reduced levels. The most likely explanation for the low plasma bradykininogen was impairment of synthesis by the cirrhotic liver, the usually normal prekallikrein levels making the other possibility of increased activation of bradykininogen to bradykinin in the plasma less likely.
