Kinematic alignment in total knee arthroplasty

Abstract: Kinematic alignment (KA) is an alternative philosophy for aligning a total knee replacement (TKR) which aims to restore all three kinematic axes of the native knee. Many of the studies on KA have actually described non-KA techniques, which has led to much confusion about what actually fits the definition of KA. Alignment should only be measured using three-dimensional cross-sectional imaging. Many of the studies looking at the influence of implants/limb alignment on total knee arthroplasty outcomes ar… Show more

“…Correction of coronal deformities requires soft tissue releases and the desire to avoid these has led to a growing interest in kinematic alignment in the recent years. Kinematic alignment aims to restore the individual pre-arthritic alignment [ 102 ]. However, complex and severe alignment deformities exist with intra- and extra-articular origins [ 21 ], and pre-arthritic alignment may differ from arthritic alignment.…”

“…The Kinematic Alignment (KA) technique for total knee arthroplasty (TKA) is an alternative surgical technique aiming to resurface knee articular surfaces. 1 , 2 Ultimately, the goal of KA is to alter the knee physiological biomechanics as little as possible by restoring native (pre-arthritic) knee joint line alignment and ligaments laxities ( Fig. 1 , ).…”

“…1 , ). 1 , 2 Setting up the orientation and height of the KA bone cuts is done by referencing the articular surfaces, by compensating for cartilage and/or bone loss, and by considering the thickness of the implants. By doing so, components are aligned on knee kinematic axes.…”

“…However, this individualized alignment is often not clearly defined, and a multitude of different philosophies, approaches, and terms can be found in the literature [ 1 ]. The clearest defined concept with the best clinical evidence available is the kinematic alignment (KA) approach [ 2 ]. The defined primary aim of KA is to reconstruct the individual joint surface of the femoral condyles with the prosthesis and thus to co-align the motion axes of the device to that of the patient’s knee [ 3 ].…”
