Kidney retrieval after sudden out of hospital refractory cardiac arrest: a cohort of uncontrolled non heart beating donors

Abstract: Introduction To counter the shortage of kidney grafts in France, a non heart beating donor (NHBD) program has recently been implemented. The aim of this study was to describe this pilot program for kidney retrieval from "uncontrolled" NHBD meaning those for whom attempts of resuscitation after a witnessed outof-hospital cardiac arrest (CA) have failed (Maastricht 1 and 2), in a centre previously trained for retrieval from brain dead donors.

“…Several European health care systems have implemented protocols to allow DCD in both of these settings, and some U.S. cities have piloted similar protocols. [4][5][6][7][8] Although emergency physicians (EPs) will likely have little involvement with inpatient DCD protocols and cases, these extended protocols directly involve EPs. Indeed, EPs will likely lead the way in implementing these protocols and even determining how widespread and accepted they are.…”

“…The relatively unknown ischemic injury and the unplanned situation of the procedure have led to a general reluctance to initiate uncontrolled DCD programs and to accept uncontrolled DCD kidneys for transplantation. There are a limited number of uncontrolled DCD kidney transplantation programs in Spain and France with excellent results [4,5]. Furthermore, a recent study shows that the clinical outcome of uncontrolled DCD kidneys is comparable to that of controlled DCD kidneys [6].…”
