Kidney Disease Improving Global Outcomes guidelines and parathyroidectomy for renal hyperparathyroidism

Abstract: Introduction Patients with end stage renal disease(ESRD) develop hypocalcemia, resulting in secondary hyperparathyroidism(SHPT). No clear criterions exist to aid in surgical decision making for SHPT. The 2009 Kidney Disease: Improving Global Outcomes(KDIGO) guidelines provide target ranges for serum calcium, phosphate and parathyroid hormone(PTH) levels in patients with ESRD. Parathyroidectomy can help achieve these targets. The study purpose is to examine how parathyroidectomy for SHPT impacts KDIGO targets d… Show more

“…MWA devices create a uniform electromagnetic field surrounding the antenna that causes rapid volume tissue heating by means of the oscillation of polar water molecules. Tissue temperatures can increase markedly, because MWA does not rely on the KDIGO guidelines range (35). The overall mortality (1%) and morbidity (3.5%) of parathyroidectomy are higher than those in our study (36).…”

“…The Kidney Disease: Improving Global Outcomes (KDIGO) guidelines recommend that screening and management of sHPT should be proposed to all patients starting CKD stage 3 [1,5]. The prevalence in Europe of sHPT among patients with requiring RRT is estimated at 43.8% in France, 46.8% in Russia, and 42.9% in the United Kingdom.…”

“…The Kidney Disease: Improving Global Outcomes (KDIGO) guidelines recommend that all patients should be screened and receive treatment for secondary hyperparathyroidism with CKD stage 3 (estimated GFR < 60 mL/min). The management of sHPT is a stepwise technique that aims to optimize serum concentrations of calcium and phosphorus, combining low phosphorus diet with pharmacological treatment [1,5,9,10].…”

“…Parathyroidectomy is the most frequently performed operation in ESRF patients [21], but there are no well-recognized guidelines for intraoperative fluid management. In this study, we aimed to optimize fluid management in patients with SPTH and ESRF undergoing parathyroidectomy.…”
