Kidney autotransplantation: long-term outcomes and complications. Experience in a tertiary hospital and literature review

Abstract: KAT is an effective treatment for complex ureteral lesions and kidney vascular abnormalities, with good results in the long term. Surgical complications are frequent, but usually minor. As a challenging surgery, it should be performed by experienced kidney transplant surgeons. Complex and proximal ureteral injuries are nowadays the main indication of this procedure.

“…In another multicenter study of 51 patients-5 of whom had undergone autotransplantation of a solitary kidney-the reported complication rates were on par or better than complication rates associated with other major urological operations [8]. The most common complications associated with KAT were urinary tract infection and hemorrhage [2,3]. Higher grade complications that were associated with KAT included septic shock and venous thrombosis [8].…”

“…Since that time, it has been used as an alternate treatment for a variety of complex renal issues. The most common indication for KAT has been for the treatment of complex and proximal ureteral strictures [3,4]. KAT itself is a relatively rare procedure.…”

“…In one study, the mean age was 41 years (range, 34 to 59 years), and none of the patients had any major or intermediate preoperative cardiac risk factors. 19 The literature indicates a propensity of LPHS to affect females more commonly than males. 8 In our study, the median age was 40.1 years, and 64% of patients were female, results that corroborate the literature.…”

“…To maintain the patient's bilateral kidney function, renal auto transplantation was taken into consideration. In fact, there are several indications for renal auto transplantation described in the current literature, including ureteral strictures and complex vascular abnormalities affecting renal arteries [25]. e procedure had been first described by James Hardy in 1963, who treated complex ureteral injuries during aortic surgery.…”
