Background and Objective: Along with erythrocytes, the Kidd glycoprotein exists in the kidney, where it enables the kidney to accumulate an elevated urea concentration, which is required for the kidney to produce concentrated urine. The primary objective of this research is to look for Kidd genes (Jk) in patients with end-stage chronic kidney disease. Materials and Methods: This is a prospective cross-sectional study of 200 patients with chronic renal failure in Khartoum in the period from June 2018 to November 2021. The patients were being monitored in clinic and the majority of them were nearing the end of their chronic kidney disease (ESCKD). Results: When the positive and negative Jk Kidd antigens in ESCKD patients were compared, it was discovered that there were significant differences in urea (P-value 0.001), creatinine (P-value 0.001) and uric acid (P-value 0.04), but no difference in sodium or potassium. Conclusion:Kidd blood group influences serum urea, creatinine and uric acid levels significantly. This effect was observed in different Kidd blood group antigens, including Jka+, Jkb+ and Jka+b+. This means that the Jk group is important in estimating the severity of ESCKD; this discovery could be useful in determining dialysis time and treatment response.

Background: Kidd (SLC14A1 rs1058396) genotyping is an important test to prevent delayed hemolytic transfusion reactions and hemolytic disease of the fetus or newborn. The PCR-restriction fragment length polymorphism (RFLP) technique using the MnlI restriction enzyme is not used widely because of the need for a polyacrylamide gel. PCR-RFLP was performed by electrophoresis with general-purpose agarose gel, and lithium borate buffer (LBB) was developed as a replacement for polyacrylamide gel. Methods: Seventy-two venous blood samples were collected randomly and used in this study. A 3% agarose gel containing 1 µg/mL of ethidium bromide and 1 mM LBB, and a high-voltage (300 V) were used to separate the short-length restriction fragments (72 bp, 51 bp, 36 bp, and 21 bp). The PCR-RFLP results were confirmed by PCR-direct sequencing. Results: Target restriction fragments could be easily discriminated. The results obtained with the PCR-RFLP were completely in agreement with the results of PCR-direct sequencing. Conclusion:The PCR-RFLP using a general-purpose agarose gel and LBB is an accurate and reliable assay for Kidd genotyping.

Τα αντιερυθροκυτταρικά αντισώματα παράγονται μετά από ευαισθητοποίηση έναντι των αντίστοιχων ερυθροκυτταρικών αντιγόνων συνήθως μέσω μετάγγισης ή κύησης. Η τιτλοποίηση των ερυθροκυτταρικών αντισωμάτων είναι μια ημι- ποσοτική μέθοδος για την αξιολόγηση της συγκέντρωσής τους και θεωρείται χρήσιμο μέσο εκτίμησης της αλλοανοσοποίησης της μητέρας κατά τη διάρκεια της εγκυμοσύνης. Παραδοσιακά, η τιτλοποίηση πραγματοποιείται χρησιμοποιώντας την κλασική μέθοδο σωληναρίου (Conventional Tube Test- CTT). Η μέθοδος που βασίζεται στη συγκόλληση με χρήση καρτών γέλης (micro column agglutination based method - GMA) έχει αποδειχθεί αξιόπιστη για πολλές δοκιμασίες στην ανοσοαιματολογία. Η μελέτη μας συνέκρινε τις μεθόδους CTT και GMA από δύο διαφορετικούς κατασκευαστές για τιτλοποίηση ερυθροκυτταρικών αντισωμάτων. Αξιολογήθηκαν δείγματα ορού με κλινικά σημαντικά ερυθροκυτταρικά αλλοαντισώματα. Κάθε δείγμα τιτλοποιήθηκε συγχρόνως με κλασική μέθοδο CTT, κάρτες γέλης ID-DiaMed-GmbH, Cressier, Switzerland (GMA1) και επίσης με κάρτες γέλης DG Gel Coombs Diagnostic Grifols, Passeig Fluvial, Spain (GMA2).Πραγματοποιήθηκαν και αξιολογήθηκαν 137 (εκατόν τριάντα επτά) τιτλοποιήσεις που αφορούσαν 50 αντισώματα αντι-D, 25 αντί-Kell, 10 anti-E, 9 anti-Jka, 8 anti-c, 5 αντι-Cw, 5 αντι-Fya, 7 αντι-Μ, 6 αντι-Kpa, 3 anti-Lua, 1 αντι-e, 3 αντι-G και 2 αντι-Cha. Τα δείγματα που ελέγχθηκαν με CTT σε σύγκριση με GMA1 και GMA2 εμφάνισαν στις περισσότερες περιπτώσεις ίσους ή υψηλότερους τίτλους με τη μέθοδο γέλης και των δύο κατασκευαστών. Στην σύγκριση τόσο CTT με GMA1 όσο και CTT με GMA2 υπήρξε καλή συμφωνία των αποτελεσμάτων (W = 0.91, p <0.0001 και W = 0.92, p <0.0001 αντίστοιχα). Για όλες τις ειδικότητες αντισωμάτων η μέση απόλυτη διαφορά κυμάνθηκε από 1 έως 3 τίτλους για αμφότερες τις μεθόδους γέλης (GMA1 και GMA2) έναντι της κλασικής μεθόδου CTT. Τα αποτελέσματα της τιτλοποίησης GMA1 σε σύγκριση με της GMA2 συμφωνούσαν σχεδόν απόλυτα (W = 0.95, p <0.0001). Συμπερασματικά παρόλο που η μέθοδος γέλης GMA και από τους δύο διαφορετικούς κατασκευαστές βρέθηκε να παρουσιάζει μεγαλύτερη ευαισθησία από την κλασική μέθοδο CTT στην τιτλοποίηση αλλοαντισωμάτων, οι διαφορές δεν ήταν σημαντικές και η συμφωνία μεταξύ όλων των μεθόδων ήταν πολύ καλή, υποδεικνύοντας ενδεχομένως την σύγχρονη μέθοδο γέλης GMA (με αποδεδειγμένη υψηλή ευαισθησία, ειδικότητα, επαναληψιμότητα και δυνατότητα αυτοματοποίησης) ως κατάλληλη εναλλακτική που θα μπορούσε να αντικαταστήσει την κλασική μέθοδο σωληναρίου CTT σε τιτλοποιήσεις αντιερυθροκυτταρικών αντισωμάτων.
