<abstract> <p>In November 2022, the global population had officially crossed eight billion. It has long been recognized that socioeconomic or health-related problems in the community always accompany an uncontrolled population expansion. International calls have been made regarding lack of universal health coverage, an insufficient supply of healthcare providers, the burden of noncommunicable disease, population aging and the difficulty in obtaining safe drinking water and food. The present health policy paper discusses how to conquer these crowded world issues, including (1) promoting government and international organization participation in providing appropriate infrastructure, funding and distribution to assist people's health and well-being; (2) shifting health program towards a more preventive approach and (3) reducing inequalities, particularly for the marginalized, isolated and underrepresented population. These fundamental principles of health policy delivery as a response to an increasingly crowded world and its challenges are crucial for reducing the burden associated with excessive healthcare costs, decreased productivity and deteriorating environmental quality.</p> </abstract>

Neonatal health is a significant global public health concern, and the first two days of life are crucial for newborn survival. Most studies on newborn postnatal care have focused on crude coverage measures, which limit the evaluation of care quality. However, evidence suggests a shift towards emphasising effective coverage, which incorporates the quality of care when measuring intervention coverage. This research aimed to assess the effective coverage of newborn postnatal care in Ethiopia while also examining its inequalities and spatial distribution. The study used secondary data from the 2016 Ethiopian Demographic and Health Survey, which was a cross-sectional community-based study. A total weighted sample of 4169 women was used for analyses. We calculated crude coverage, which is the proportion who received a postnatal check within 48 hours of birth and quality-adjusted coverage (effective coverage), which is the proportion who received a postnatal check within 48 hours of birth and reported receipt of 6 or more contents of care provided by health care providers. Concentration index and concentration curves were used to estimate the socioeconomic-related inequalities in quality-adjusted newborn postnatal care. The spatial statistic was analysed by using Arc-GIS. The crude coverage of newborn postnatal care was found to be 13.2%, while the effective coverage was 9%. High-quality postnatal care was disproportionately concentrated among the rich. A spatial variation was found in quality-adjusted coverage of newborn postnatal care across regions. The findings suggest that there is a significant gap in the coverage and quality of postnatal care for newborns across regions in Ethiopia. The low rates of coverage and effective coverage, combined with the concentration of high-quality care among the rich and the spatial variation across regions, highlight the need for targeted interventions and policies to address the inequalities in access to high-quality postnatal care for newborns.

In complex systems constrained by multiple factors, it is very important to identify the key influencing factors for mastering the evolution and development law of a system and for obtaining scientific decision-making suggestions or schemes. At present, the method based on experimental simulation is limited by the difficulty of system model construction; DEMATEL (Factual Decision Trial and Evaluation Laboratory) is inevitably influenced by subjective factors. In view of this, we propose a novel model based on heuristic causal inference. By combining the network analysis in complex network science, the model defines the global/local causal pathway and the causal pathway’s length in the causal network and takes the causal pathway contribution degree as an indicator to measure the approximate causal effects. The model includes steps such as causal network learning, causal pathway contribution degree calculation, and key influencing factor identification. The model uses the Fast Causal Inference (FCI) algorithm with prior knowledge to learn the global causal network of the complex system and uses the heuristic causal inference to calculate the causal pathway contribution degree. The heuristic method draws on the idea of complex network topology analysis and measures the influence degree between variables by the number and distance of causal pathways. The key influencing factors are finally identified according to the causal pathway contribution degree. Based on the SECOM dataset, we carried out simulation experiments and demonstrated the feasibility and effectiveness of the proposed method.
