Keratosis punctata of the Palmar Creases: Case Report and Prevalence Study in Caucasians

Abstract: Keratosis punctata of the palmar creases (KPPC) is a variant of keratosis punctata of palms and soles in which lesions are confined to palmar creases. We present a patient with KPPC and study the prevalence of the disease in Caucasians. The survey included 1,001 white patients, that were examined for palmar lesions. We could not find any case that fulfilled the diagnostic criteria for KPPC. We conclude that KPPC is a rare disease among Caucasians but rather common in the black race.

“…Keratosis punctata of palmar creases is considered to be a variant of punctate keratoderma, in which the lesions are confined to palmar creases [12]. The presence of a dilated and tortuous acrosyringium, as well as a history of deterioration of lesions with water exposure, helped us to exclude the diagnosis in this instance.…”

“…M adam , Several pieces of evidence indicate that certain types of palmoplantar keratoderma (PPK) occur predominantly in black patients 1–6 . These comprise two relatively frequent diseases: keratosis punctata of the palmar creases (KPPC) 1–4 and focal acral hyperkeratosis (FAH), which is now considered to be a variant of acrokeratoelastoidosis 5,6 . Other related conditions that may preferentially affect black people are ainhum, knuckle pads (KP) 6 and the very rare mosaic acral keratosis.…”

“…Shmunes and Wood (1) reported a case of SCN in a boy with ichthyosis, which is due to reduced eccrine sweating; however, we could not find any cases of SCN occurring in a hyperhidrotic individual. Keratosis punctata has been reported to OCcur in association with different diseases (4,5) or hard manual labor (6)(7)(8)(9)(10)(11). There have been several reports on the presence of eccrine sweat ducts within the lesions (10,11).…”
