]3ekanntlich hat SC~LOFF~g die ausgiebige Mitentfernung der I-Iaut bei der Radikaloperation des Mammacareinoms gefibt und in Wort und Schrif~ gefordert. Seine geringe Zahl an lokalen Rezidiven ist bis heute kaum yon anderer Seite iibertroffen women. Seine diesbezfigliche Arbei~ (1930)stfitzt sieh auf das Krankengut der Prager Chirurgischen Universit~tsklinik; als damaliger Assistent SCaLOF~E~s habe ich die einschl~gigen Nachuntersuchungen durchgeffihrt. Dabei habe ieh eine Reihe anderswo operierter Mamm~carcinom-Patienten zu Gesicht bekommen, die nicht in der erw~hnten Arbeit aufseheinen. Unter ihnen sind mir 2 Patientinnen auigefallen und wegen der eiger~tiimlichen Lokalisierung des 6rtliehen l~ezidivs ~n Erinnerung geblieben. Bei beiden lieB die Spannung der Haut und die Form der Narbe daraui schliel]en, dal] der vorgenommene prim~re WundverschluB erzwungen worden war und dab deshalb die Hautn~hte an einzelnen Stelle~ tier eingeschnitten bzw. durchgesehnitten hatten. Das umschnLirte Gewebe war offenbar nekrotisch geworden. In den Stichkan~len jener Hautn/~hte, die offenkundig unter Spannung gestanden hatten und zwar nut in diesen, fanden sich Tumorknoten. Sonst war nirgend wo anders ein Rezidiv oder eine Metastase festzustellen. Die landl~ufige Erklgrung, dab es sieh dabei um Implantationsmetastasen in den Stichkan/~len dutch die Nadel bzw. den Faden handle, h~t reich nieht befriedigt, da mir, wenn ich mir den Ablauf der Operation vorstellte, der Gang der Implantation nicht recht einleuch~en wollte.Wo solite sich Nadel und Faden mit Carcinomzellen beladen haben, wenn iln Zeitpunkt der ttautnaht das Careinom bereits radikal ausgesehnitten war ? Ich muBte annehmen, dab man den Tumor tatsgchlieh bereits radikal entfernt hatte; w~ren Tumorreste in der Wunde zuriickgeblieben und hgtten AnlaB zur Infektion des Fadens mit Tumorzellen gegeben, dana wi~ren vermutlich diese Reste limgst zu makroskopisch sichtbaren Rezidiven im Operationsgebiet herangewachsen, was aber nicht der Fall war, denn, wie schon erw~hnt, land sieh nut in den Stichkanglen ein nachweisbares Lokalrezidiv.Man k6nnte daran denken, dal~ die carcinomatSse Infektion des Nahtmaterials durch die Gummihandschuhe iibertragen worden sei, die
