Objectives: In this study, we aimed to compare the efficacy of ultrasonography (US) and steroid phonophoresis (PH) treatments in patients with idiopathic carpal tunnel syndrome (CTS). Patients and methods: Between January 2013 and May 2015, a total of 46 hands of 27 patients (5 males, 22 females; mean age: 47.3+13.7 years; range, 23 to 67 years) with idiopathic mild/moderate CTS without tenor atrophy or spontaneous activity in abductor pollicis brevis were included. The patients were randomly divided into three groups. The first group was ultrasound (US) group, the second group was PH group, and the third group was placebo US group. Continuous US with a frequency of 1 MHz, an intensity of 1.0 W/cm2 was used in the US and the PH groups. The PH group received 0.1% dexamethasone. Placebo group received a frequency of 0 MHz, an intensity of 0 W/cm2 US. Treatments were administered for five days a week, a total of 10 sessions. All patients also wore night splints during treatment. The Visual Analog Scale (VAS), Boston Carpal Tunnel Questionnaire consisting of two parts, namely the Symptom Severity Scale and Functional Status Scale), grip strength, and electroneurophysiological evaluations were compared before the treatment, after the treatment, and three months later. Results: All clinical parameters improved in all groups after treatment and at three months, except for the grip strength. Recovery in the sensory nerve conduction velocity between palm and wrist was seen in US group at three months after the treatment; however, recovery in the sensory nerve distal latency between the second finger and palm was seen in PH and placebo groups after treatment and at three months after the treatment. Conclusion: The results of this study suggest that splinting therapy combined with steroid PH, placebo or continuous US is effective for both clinical and electroneurophysiological improvement; however, electroneurophysiological improvement is limited.

The aim of this study was to investigate and compare the effectiveness of splinting and Kinesio taping (KT) as forms of treatment for patients with mild to moderate idiopathic carpal tunnel syndrome (CTS) by evaluating the symptoms and measuring the range of function and electrophysiological findings. This single-blind, randomized, prospective study included 44 patients (58 hands) suffering mild to moderate idiopathic CTS according to nerve conduction studies (NCS). The patients were randomized into two groups and evaluated before treatment and 6 weeks after treatment by means of physical examination findings, NCS, and the Boston Carpal Tunnel Questionnaire (BCTQ) functional status and symptom severity scale scores. When the differences in both groups before and after treatment were compared, a significant improvement was observed in the KT group compared to the splint group in terms of electrophysiological changes, provocative test responses, BCTQ symptom severity, and functional status scores. KT may help prevent the disease from progressing further in mild and moderate idiopathic CTS when applied in time using the appropriate technique and be a good alternative to neutral splinting in terms of patient compliance.

Carpal tunnel syndrome (CTS) is the most common compression neuropathy, affecting nearly 5% of the general population. The modern approach to its treatment is complex and includes early diagnosis, prevention and treatment of all conditions and diseases leading to its manifestation. Many authors have worked on the problems related to the possibilities of physical therapy in treating CTS, but the question of optimal treatment and rehabilitation remains relevant to this day. In practice, various rehabilitation programs are constantly being implemented and developed. Many have demonstrated positive outcomes, but work in this area persists. Every novel approach or integration of established physiotherapeutic and rehabilitation techniques enhances the current methods and expands the potential for more comprehensive functional recovery of the impaired limb, as well as the patient's restoration to their previous lifestyle. Physical therapy has advanced significantly in recent decades due to the introduction of numerous new techniques and technologies. The article offers a brief literature overview of several contemporary physical medicine approaches that can be applied in the conservative treatment of carpal tunnel syndrome based on their physiological and therapeutic effects. Modern physical therapy approaches can be a viable alternative to established physical therapy methods like ultrasound, low-level laser, iontophoresis, magnetotherapy, TENS, and others. It is crucial to conduct additional qualitative and comprehensive investigations to verify the effectiveness of physical therapy in treating CTS, identify the most efficient ways, and create improved protocols for its conservative treatment.
