<p class="abstract">The lips are major aesthetic components of the face, which are also necessary for facial expression, speech, and eating. In oncological resections, the main goal in lip reconstruction is achieving oral competence than speech and facial expressions. Malignant lesions involving lip warrant a wide excision to ensure a disease-free margin, which usually results in large defects. Defects up to 1/3rd of lips are closed primarily. Defects measuring 1/3rd to 2/3rd of the lower lip may be closed with Karapandzic, Abbe or Estlander flaps. A 45-year-old male presented with an exophytic lesion in the lower lip involving the facial skin with bilateral level 1b cervical lymphadenopathy. Contrast-enhanced computed tomography scan showed heterogeneous irregular lesion over the lower lip with bilateral enlarged necrotic level 1b cervical lymphadenopathy. Biopsy from the lesion was suggestive of moderately differentiated squamous cell carcinoma. Wide local excision with left modified radical neck dissection and right supraomohyoid neck dissection was done. The central lower lip defect was reconstructed with bilateral Karapandzic flap. The postoperative period was uneventful, although the patient had microstomia. The patient was advised adjuvant radiotherapy based on histopathology and was in regular follow up.</p>

Background: The lip has functional and aesthetic importance. Lip defects occur due to the variety of etiology and the choice of their reconstruction has profound effect on functions and cosmesis. There are multiple options available for reconstruction according to defect size, but superiority of one method over another is still debated and hence the methods and their outcome were analyzed prospectively. Material and method: Twenty-one patients with all sizes and locations of defects in upper and lower lip with acquired etiology were included in the evaluation. Reconstruction was performed according to defect size, availability of local/regional and distant donor tissue, defect location, patients’ comorbid conditions and patients’ preference. Patients were assessed at 1 month and 6 months postoperatively. Observers’ and patients’ input were also taken into account for outcome. Results: Out of 21 patients, 5 free radial artery forearm flap reconstructions, 4 nasolabial flap reconstructions, 5 primary closures of defect, 4 Estlander flap reconstructions, 2 lip advancements, and one Karapandzic flap reconstruction were done. Free flap and nasolabial flap had hypoesthesia and incompetence if commissure is reconstructed and problem of bulk, restricted mobility and vermilion mismatch. Local and lip flaps were associated with decreased stoma size and some form of local scarring and asymmetry. However, all patients were satisfied with the functional and aesthetic outcome. Conclusion: Local flaps are better in terms of functional and aesthetic outcome but with some degree of microstomia which was well tolerated by most patients. Regional and distant flaps provide reconstruction where no other option is available and provide good functional support and acceptable cosmesis.

The incidence and prevalence of lip and oral cavity cancer has increased over the last decade, worldwide and in India. It ranks at 1st position in males in india, all ages, in 2020. Lips are essential organ of the body which carries dynamic role in facial expression, speech, sensuality, deglutition. Resection of central, large, lower lip cancer creates a more than 2/3rd large defect. Covering of this defect with maintenance of oral competency is a difficult task for a treating surgeon. Number of techniques are described for covering of large lower lip defects. Out of these techniques, karapandzic flap is a successful, accepted, simple, easy to learn reconstructive procedure with good cosmetic results. It is a modification of Gillie’s fan flap and it involves unilateral or bilateral full-thickness circumoral advancement-rotation flaps. The feature which distinguishes it from other techniques is preservation of neuro-vascular integrity, symmetry and oral competency. We are reporting a case of an elderly gentleman with large, central, lower lip cancer who underwent oncological resection with reconstruction by karapandzic flap technique.
