In this prospective, randomized controlled trial, changes in endotracheal tube cuff pressure were studied in 60 patients undergoing elective surgery under general anaesthesia with nitrous oxide and oxygen. The cuffs were inflated with either air or distilled water. The mean pressure in the air-filled cuffs increased steadily throughout the procedure, reaching 47.5±7.3 cmH 2 O at one hour compared with 31.6±2.4 cmH 2 O mean pressure in the water-filled cuffs. The pressure and the rate of rise in cuff pressure were significantly lower (P<0.05) in the water-filled cuffs throughout the hour of study. When an endotracheal tube cuff is distended with water, the rise in cuff pressure during nitrous oxide anaesthesia is lower than that of an air-filled cuff.

SummaryChanges in cuff pressure and volume of tracheal tubes were studied in 60 patients undergoing lower abdominal surgery under generol anaesthesia with nitrous oxide and oxygen as the anaesthetic gas mixture. The cuffs were inflated with either anaesthetic gas mixture or air. Three different brands of tubes were used. The pressure in the air filled cuffs increased steadily throughout the procedure and reached level high enough to impede microcirculation in the tracheal mucosa within one hour. We conclude that filling the cuffs with anaesthetic gas mixture is a simple and reliable way to achieve stable cuff pressure during anaesthesia.

SummaryA constant pressure differential valve for the control of tracheal tube cuff pressure was tested under clinical conditions. Fifty-one patients underwent controlled ventilation and 20 patients were allowed to breathe spontaneously. Nitrous oxide 66% with oxygen 33% and halothane were used via a circle system. With controlled respiration at afresh gas flow of 3-10 Imin -', the expiratory cuff pressures of 10.1-16 cmH20 and the inspiratory cuffpressures of 23.4-32.4 cmH,O were below venous and arterial mucosal capillary perfusion pressures respectively. Cuff pressures were unaltered with time. Methylene blue instilled into the larynx did not appear in the trachea. Fifty-two control patients had the same incidence of sore throat (40%) and hoarseness (30%) at 24 h. With spontaneous ventilation, fresh g a s j o w s of 5-15 Imin-' maintained the cuff pressure above 10 cmH20. We conclude that this valve prevents excessive tracheal cuff pressure while maintaining the airway seal.
