Objetivo: verificar a influência de uma abordagem fisioterapêutica multimodal na dor, qualidade de vida, mobilidade e força de preensão palmar em pacientes com sequelas do vírus Chikungunya (CHIKV). Método: a amostra foi composta por 21 indivíduos com artralgia persistente após infecção pelo CHIKV. Foram coletados dados sociodemográficos, número de locais de dor, intensidade da dor pela escala visual analógica (EVA), mobilidade pelo teste timed up and go (TUG), qualidade de vida pelo questionário SF-36 e força por dinamometria manual. A abordagem consistiu em mobilizações articulares, alongamentos, exercícios aeróbicos, ativo-resistidos, ativo-livres e recursos eletrotermoterapêuticos, totalizando dez sessões. Resultados: após a intervenção, houve redução do número de locais (8,23 para 4,38, p=0,0001; Z = 3,8912) e da intensidade da dor (EVA: 5,86 cm para 3,14 cm, p=0,0001; Z = 3,8912), do tempo no TUG (12,38 s para 10,29 s, p=0,0008; Z = 2,0237), aumento dos domínios relativos ao aspecto físico do SF-36 e da força de preensão palmar direita (17,04 Kg para 18,74 Kg, p=0,0324; Z = 2,1272). Conclusão: uma abordagem terapêutica multimodal pode reduzir a dor, aumentar a mobilidade, qualidade de vida e força de preensão palmar em indivíduos com sequela do CHIKV.

Objectives. The purpose of this study was to evaluate the impact of manual therapy on the management of rheumatoid arthritis (RA) patients with knee pain. Materials and Methods. This was a small, randomized clinical pilot study. Subjects were 46 patients with diagnosed RA, randomly assigned to the manual therapy group (postisometric relaxation and joint mobilization) or control group (standard exercise). Subjects in each group had 10 sessions of interventions, once a day with one day break after the sixth day. Outcomes included the pain intensity of knee, Knee Society Score, Oxford Knee Score, and Health Assessment Questionnaire. Results. There were no statistically significant differences between groups, except for the pain intensity of the knee. Conclusions. This study suggests that manual therapy (postisometric relaxation and joint mobilization) may have clinical benefits for treating knee pain and function in rheumatoid patients. Further extended studies are expected to determine the effectiveness of manual therapy in RA patients with knee pain.

Outcomes of Pyrocarbon Arthroplasty in Metacarpophalangeal Joints Affected by Rheumatoid Arthritis

Background: Rheumatoid arthritis (RA) affecting the metacarpophalangeal (MCP) joint may warrant arthroplasty. The purpose of this study was to investigate implant survivorship, complications, radiographic outcomes, and clinical outcomes in patients undergoing MCP arthroplasty with a pyrocarbon implant to treat RA. Methods: In all, 124 MCP joint pyrocarbon arthroplasties in 40 patients performed to treat RA were reviewed. Operations were at the index (n = 43, 35%), middle (n = 33, 27%), ring (n = 27, 22%), and small (n = 21, 17%) fingers from 1998 to 2009 in 105 (85%) female and 19 (15%) male joints with a mean age of 54 ± 11 years. Mean postoperative follow-up was 6 ± 3 years. All patients achieved at least 2 years of follow-up. Results: Rates of implant survivorship at 1, 2, 5, and 10 years were 98%, 98%, 90%, and 81%, respectively. Fifteen percent (n = 18) of arthroplasties underwent revision at a mean 5 ± 3 years postoperatively. The overall reoperation rate was 29% (n = 36). Rates of survival from reoperation at 1, 2, 5, and 10 years were 85%, 84%, 76%, and 68%, respectively. Complications occurred in 32% (n = 40). Pain ratings improved postoperatively ( P < .01). Arc of motion improved from 37 ± 21 to 43 ± 19 ( P = .03). Both appositional and oppositional strength improved after surgery; however, there was no improvement in grip strength ( P < .01). Conclusions: Metacarpophalangeal arthroplasty with a pyrocarbon implant demonstrated reliable improvement in pain and arc of motion in patients with RA. Complication and overall reoperation rates were high, while 1 in 10 undergo revision within 5 years postoperatively.
