iTACTIC – implementing Treatment Algorithms for the Correction of Trauma-Induced Coagulopathy: study protocol for a multicentre, randomised controlled trial

Abstract: BackgroundTraumatic injury is the fourth leading cause of death globally. Half of all trauma deaths are due to bleeding and most of these will occur within 6 h of injury. Haemorrhagic shock following injury has been shown to induce a clotting dysfunction within minutes, and this early trauma-induced coagulopathy (TIC) may exacerbate bleeding and is associated with higher mortality and morbidity. In spite of improved resuscitation strategies over the last decade, current transfusion therapy still fails to corre… Show more

“…There is considerable interest in exploring the value of viscoelastic point‐of‐care (POC)‐guided haemostatic management in general trauma, but there are few data in patients with TBI . Timely treatment of coagulopathy in patients with TBI may prevent secondary injury due to bleeding and could reduce the associated disability and mortality .…”

“…Obstetric hemorrhage and the use of viscoelastic methods have been covered elsewhere by International Society on Thrombosis and Hemostasis guidance . The use of viscoelastic methods in trauma is currently still under evaluation in clinical trials (the largest being iTACTIC, due to report in 2018) and will not be covered by this guidance . The nature of this review excludes coverage of the use of platelet mapping cartridges.…”

“…The outcome benefit of factor concentrates, however, needs to be demonstrated in well-powered prospective trials, which is complicated by the legacy of its use, by the difficulty of ethically accurate study planning, and institutional inconsistency of therapeutic procedures. Data from the European multicenter iTACTIC study on coagulation management in traumatic major hemorrhage are awaited soon and may provide further insights [62].…”
