Izotiazolinony to środki konserwujące, które wchodzą w skład kosmetyków, artykułów chemii gospodarczej oraz wielu produktów przemysłowych. Początkowo stosowano mieszaninę metylochloroizotiazolinonu i metyloizotiazolinonu (MCI/MI-Kathon CG), a następnie-od początków XXI w.-wyłącznie metyloizotiazolinon (MI). Ze względu na znaczący wzrost liczby kontaktowego zapalenia skóry spowodowanego alergią na ten związek wycofano go z kosmetyków pozostawianych na skórze (leave-on) oraz zmniejszono stężenie w kosmetykach zmywalnych (rinse-off). Celem pracy była analiza częstości występowania alergii kontaktowej na izotiazolinony u pacjentów Instytutu Medycyny Pracy im. prof. J. Nofera w Łodzi, nasilenia wyników testów płatkowych, nadwrażliwości w zależności od stanowisk pracy oraz ocena udziału alergii natychmiastowej w powstawaniu uczulenia. Materiał i metody: U 1137 chorych (832 kobiet i 305 mężczyzn) z podejrzeniem kontaktowego zapalenia skóry, diagnozowanych w latach 2014-2018, przeprowadzono serię testów płatkowych, zawierających m.in. metylochlorotiazolinon/metyloizotiazolinon (0,01%) oraz metyloizotiazolinon (0,2%) (Chemotechnique Diagnostics, Szwecja). U 29 chorych uczulonych kontaktowo na izotiazolinony wykonano punktowe testy skórne z metyloizotiazolinonem (0,001%, 0,01%, 0,1%, 1%) (Sigma-Aldrich, Niemcy). Wyniki: Wśród 1137 badanych alergię na izotiazolinony potwierdzono u 95 (8,3%) chorych: 71 (8,5%) kobiet i 24 (7,9%) mężczyzn. Uzyskano 150 dodatnich wyników testów płatkowych z izotiazolinonami (13,2%). Liczba dodatnich wyników tylko z metyloizotiazolinonem była wyższa (7,8%) niż z metyloizotiazolinonem pochodzącym z Kathonu CG (5,4%). U osób uczulonych tylko na metyloizotiazolinon silnie dodatnie wyniki testów płatkowych (+++) dotyczyły aż 60,7% badanych. W grupie kobiet przeważały pracownice biurowe (42,3%) i personel medyczny (18,3%), w grupie mężczyzn-zatrudnieni na stanowiskach ślusarzy, tokarzy, mechaników i podobnych (45,8%) oraz pracownicy budowlani (25,0%). Testy punktowe z metyloizotiazolinonem były ujemne, pozytywizowały się po 3-5 godz. Wnioski: Izotiazolinony, zwłaszcza metyloizotiazolinon, są istotnymi czynnikami przyczynowymi uczulenia kontaktowego, w tym o pochodzeniu zawodowym. Mają silne właściwości alergizujące, w testach płatkowych dają często bardzo nasilone odczyny zapalne. Zagadnienie udziału alergii natychmiastowej w patogenezie uczulenia na izotiazolinony wymaga dalszych badań.

The Expert Panel for Cosmetic Ingredient Safety (Panel) reassessed the safety of Methylisothiazolinone, which functions as a preservative in cosmetics. The Panel reviewed relevant animal and human data provided in this safety assessment, and data from the previously published safety assessments of Methylisothiazolinone, and concluded that Methylisothiazolinone is safe for use in rinse-off cosmetic products at concentrations up to 100 ppm (ie, 0.01%) and safe in leave-on cosmetic products when they are formulated to be nonsensitizing, which may be determined based on a quantitative risk assessment or similar methodology.

These may suffer from airborne ACD by the emissions of freshly painted walls, in which MI is detectable for many weeks, leading to problems for the sensitised inhabitants [110][111][112], which may be protracted to almost one year [113]. While photoallergic contact dermatitis is not in the scope of this review, it has to be noted that several patients sensitised to MI displayed photo-aggravated dermatitis, both clinically, and in the patch test, where a positive, but weaker reaction than after additional UV exposure was noted [114].…”

Contact allergy (sensitisation) and allergic contact dermatitis (ACD) resulting from it have a considerable public health impact. For the present review, all pertinent articles were systematically searched via Medline and Web of Science™; additionally, all available issues of the journals “Contact Dermatitis” and “Dermatitis” were manually searched, covering the years 2018–2019, thereby extending and re-focusing a previous similar review. New allergens, or previously described allergens found in a new exposure context or of other current importance, are described in sections according to substance classes, e.g., metals, preservatives, fragrances. As a common finding in many investigations, a lack of information on product composition has been noted, for instance, regarding a newly described allergen in canvas shoes (dimethylthiocarbamylbenzothiazole sulfide) and, most notably, absence of co-operation from manufacturers of glucose-monitoring devices and insulin pumps, respectively. These latter devices have been shown to cause severe ACD in a considerable number of diabetic patients caused by the liberation of isobornyl acrylate and N,N’-dimethylacrylamide, respectively, as demonstrated by an international collaboration between dermatologists and chemists. Improved and complete ingredient labelling for all types of products, and not just as we have with cosmetics at present (apart from full listing of fragrance substances) in Europe, must be put on the legislative agenda.
