Isolated renal pelvis rupture secondary to blunt trauma: Case report

Abstract: HighlightsA case, with isolated renal pelvis rupture due to blunt abdominal trauma.Clear urine output from the urinary catheter was seen in patient.Diagnosed a large rupture of renal pelvis by abdominal contrast enhanced CT.Isolated rupture of the renal pelvis causes delays in the diagnosis of the rupture.Only a few isolated cases of pelvis rupture with resultant extravasation have been described previously in literature.Surgical intervention should be warranted in emergency cases.

“…If there is significant urinary extravasation noted, interventional radiology could place a drain. In the event of emergent collecting duct disruption, surgical intervention is required, especially in the setting of trauma [8].…”

“…CT urogram is the most sensitive diagnostic modality for renal collecting system rupture [ 11 ]. Classically, a renal pelvis rupture would reveal perinephric stranding and contrast extravasation along the perinephric, peripelvic, or retroperitoneal spaces [ 8 ]. Although a non-contrast CT was performed in this patient, imaging showed contrast extravasation from the retained IV contrast used in a previous CTA.…”

“…Currently, renal pelvis rupture causes urinoma or local inflammation and it is successfully treated by conservative methods with ureteral stent implantation [8], but surgical complication like peritonitis and septic shock can occur, so exploratory laparotomy and surgical intervention should be warranted in emergency cases [9]. Our patient was treated with ureteral stent by interventional radiology without complications and then addressed to medical therapy for the treatment of lymphoma.…”

“…Isolated renal pelvis injury is uncommon and associated with urolithiasis, iatrogenic injury, trauma, and neoplasm. [1][2][3] The injury is often associated with underlying congenital renal disease, typically unrecognized before the trauma. Ultrasonography is moderately sensitive and highly specific for renal injury, and is a useful initial diagnostic tool for patients with moderate pretest probability of injury.…”
