Isolated Pulmonary Valve Endocarditis

Abstract: Endocarditis of only the pulmonary valve is a very rare finding and is often missed during echocardiographic evaluation due to limited views of the pulmonary valve and a low index of suspicion. We report 2 cases of pulmonary valve endocarditis (PVE), highlighting the importance of echocardiography in the assessment of the infected pulmonary valve. In addition, we review the published case reports of isolated PVE from 1979 to 2013 in order to study the role of echocardiography in the diagnosis of pulmonary valv… Show more

“…Secondly, PVE is also very rare and typically occurs in 1.5–2% of patients hospitalized with IE (5). It is usually associated with simultaneous involvement of other valves (6), commonly in the context of an underlying structural heart disease. A total of 70 cases of isolated PVE have been reported between 1979 and 2013 (6).…”

“…However, isolated PVE is an extremely rare event that typically occurs in less than 2% of cases (1). From 1979 to 2013, there were only 70 reported cases of PVE (2). The mean age at presentation was 44 years and the majority of patients were male.…”

“…However, cases of fungal as well as other bacterial pathogens have also been reported in the literature. TTE is the most common modality used in the diagnosis of PVE and often requires multiple views to pick up the vegetation (2). In cases where TTE is negative despite high clinical suspicion, a transesophageal echocardiogram should be performed because of its better sensitivity and specificity.…”

“…1,2 Isolated PV endocarditis is very rare, accounting for less than 2.0% of hospitalizations for IE, and is extremely rare in patients with structurally normal hearts. [2][3][4] It mostly affects men, 5 and its main risk factors include intravenous drug abuse, alcohol abuse, sepsis, catheter -related infection, immunosuppression, dental extraction, and bowel surgery.…”
