Isolated popliteus muscle rupture in polo players

Abstract: Isolated rupture of the popliteus muscle seems to be extremely rare, with only two cases described in the English literature. We present two cases of magnetic resonance imaging-verified isolated rupture of the popliteus muscle in polo players with the same injury mechanism-external rotation trauma on a flexed knee. Functional testing of the popliteus muscle in the figure of four position was positive in both cases. Full recovery was observed after 6 weeks of physiotherapy.

“…Only 8% of all popliteus injuries occur in an isolated fashion, with isolated cases of muscle or tendon lesions reported in skiers, 55 football, [55][56][57] soccer, 58,59 rugby, 60 and polo players. 61 Lesions of the popliteus muscle are usually combined with injuries to other knee structures. 13 Brown and colleagues, 54 in their MRI analysis of popliteus injuries (n 5 24), reported combined injuries in 92% of cases, with involvement of its muscular portion in 96% of patients.…”

“…Other injuries in polo reported in the literature include Winge and Phadke's article that describes two cases of isolated rupture of the popliteus muscles in polo players as they externally rotated on a flexed knee (11). This is a very rare injury, with only two cases from any cause being reported before their article.…”

“…Tables 2 and 3 provide injury and treatment information for the included studies. Initial radiography was performed on 74% (n ¼ 20/27) of injuries, followed by MRI 1,2,9,11,13,33,36,37 or diagnostic arthroscopy 8,10,21,22 to confirm the diagnosis of an isolated popliteus injury. Injuries most commonly involved avulsions of the popliteus tendon from the lateral femoral condyle (67%; n ¼ 18/27), followed by intrasubstance muscular or tendonous tearing (30%; n ¼ 8/27) 4,5,9,20,24,28,37 or tearing at the musculotendinous junction (4%; n ¼ 1/27).…”
