Isolated IgG4-related sclerosing cholangitis misdiagnosed as malignancy in an area with endemic cholangiocarcinoma: a case report

Abstract: BackgroundThe most common cause of perihilar obstruction is cholangiocarcinoma, especially in Thailand. Benign perihilar stricture represents less than 20% of cases. IgG4-related disease and IgG4-related sclerosing cholangitis, however, have been receiving increased recognition. Isolated IgG4-related sclerosing cholangitis is less common. The preoperative diagnosis of IgG4-related sclerosing cholangitis without pancreatic involvement is very difficult because the clinical presentation and preoperative evaluati… Show more

“…Whereas PSC and cholangiocarcinoma may be treated through surgical intervention, ISC is treated with a medical approach, specifically high dose steroids. It thus is important to consider, and could have prevented unnecessary surgery in various case reports of patients wrongly diagnosed with other causes of biliary pathology 4 5…”

“…There are some characteristic image findings in computed tomography and MRI that are useful for the diagnosis of IAC, including multifocal biliary strictures, a smooth outer margin, a markedly thickened wall of bile duct, or a narrow but visible lumen. [ 23 ] And we will find hyperenhancement during the late arterial phase, homogeneous hyperenhancement during the delayed phase, concurrent gallbladder wall thickening, and lack of vascular invasion. [ 23 27 ]…”

“…The rate of benign strictures misdiagnosed preoperatively as cholangiocellular carcinoma is high, variant data can be found in the literature: about 7-18%, even up to 50% [4,15,16]. The term 'malignant masquerade' was first used in 1985 by Hadjis et al, they described the malignant-behaving, but histologically proven benign bile duct strictures with this expression, the lesions of which cannot be distinguished by clinical or imaging features [17].…”
