Isolated Corticotropin Deficiency in Adults

Abstract: The clinical and laboratory findings in 76 patients with isolated corticotropin deficiency (10 of our own and 66 from literature) were analyzed with the following observations. With the exceptions of hyperpigmentation and hyperkalemia, the similarity of symptoms and signs to those of Addison's disease and their reversibility by glucocorticoids indicate that most, but not all, manifestation of isolated corticotropin deficiency is caused by glucocorticoid deficiency. Isolated corticotropin deficiency seems to be… Show more

“…The principle signs and symptoms associated with isolated ACTHdeficiency are variable, non-specific and similar to those seen in adrenocortical insufficiency due to other causes (8). The present patient showed the typical symptomsof isolated ACTHdeficiency, such as general malaise, gastrointestinal symptoms, the loss of mental activity, and hypotension (8,9). However, other endocrinological disorders such as thyroid dysfunction or hypoglycemia, which frequently occur in association with this disease, were not found.…”

“…Isolated ACTH deficiency is a secondary adrenal insufficiency that may become life-threatening [3,4]. Only ACTH among six anterior pituitary-hormones is not secreted.…”

“…Isolated ACTH deficiency is a cause of secondary adrenocortical insufficiency [3,4]. Conspicuous symptoms of IAD are severe general malaises, appetite loss and weakness reflecting adrenal insufficiency and joint stiffness is also considered to be related to IAD [5].…”

“…However, increased thyroid‐stimulating hormone (TSH) levels were reported in 45 of 76 patients recently reviewed. 25 The hypersecretion of TSH is not due to a decrease in negative feedback, since these patients do not have subnormal levels of thyroid hormones, but may instead result from increased production of its hypothalamic secretagogue thyrotropin releasing hormone (TRH). While TRH itself does not affect ACTH secretion, we hypothesized that another peptide derived from the preproTRH precursor and co‐released with TRH could have CRIF activity.…”
