Isolated Bone Marrow Manifestation of HIV-Associated Hodgkin Lymphoma

Abstract: Human immunodeficiency virus-associated Hodgkin lymphoma frequently involves the bone marrow and is usually recognized at staging after Hodgkin lymphoma diagnosis on a lymph node or other tissue biopsies, but occasionally the marrow involvement is the only apparent manifestation of disease. In the latter setting, diagnosis can be problematic. From a total of 42 patients with newly diagnosed human immunodeficiency virus-associated Hodgkin lymphoma, 22 subjects had positive marrow involvement at diagnosis; 16 of… Show more

“…In the seven previously reported PBMHL cases, RSCs were positive for EBER or EBV-latent membrane protein (LMP)-1, as also observed in our patient (2,4,5,7,8). LMP-1 is found in all cases of HIV-associated PBMHL, highlighting the role of LMP-1 in its pathogenesis (12,13).…”

“…It is very difficult to diagnose PBMHL, even for pathologists specializing in lymphoma, as typical RSCs are absent (10) and only a few of these cells are found in the histological specimens (2,6). In order to diagnose PBMHL, it is necessary to exclude the possibility of EBV-positive DLBCL of the elderly.…”

“…In our experience and that of others, bone marrow may often be the initial site of disease presentation. 52 More recently, we extracted DNA from BMT specimens processed by our protocol for the purposes of PCR amplification. We carried out PCR analysis for immunoglobulin gene heavy-chain rearrangement on a few samples ( fig 5).…”

“…In only 2 patients, bone marrow was the only site of disease at diagnosis. Ponzoni et al 7 have recently reported a series of 6 HIV-infected patients with isolated bone marrow involvement. Isolated bone marrow HIV-associated Hodgkin's lymphoma may be an underestimated condition in HIV-infected patients.…”
