Abstract:We report a case of an uncommon injury: a combined avulsion fracture of the greater and lesser tuberosity in the proximal humerus. Standard radiographs permitted the diagnosis and CT scans confirmed the absence of a shoulder dislocation. The fracture of the tubercles was treated by internal fixation. At 24 months of follow-up, the patient was able to walk without pain and radiographic result was excellent.

Le fratture della grande tuberosità (trochite) e della piccola tuberosità (trochine) dell'omero nella maggior parte dei casi sono associate a fratture complesse comminute dell'omero prossimale [1]. Più raramente si presentano come fratture isolate e hanno specifiche peculiarità, sia nel meccanismo lesivo, sia nella diagnosi, sia nel tipo di trattamento [1][2][3]. A differenza delle fratture complesse a 3 o 4 frammenti dell'omero prossimale, che colpiscono tipicamente persone anziane, osteoporotiche, prevalentemente di sesso femminile, le fratture isolate delle tuberosità omerali avvengono più frequentemente in pazienti maschi tra la 2 a e la 5 a decade di vita e negli adolescenti con la fisi dell'omero prossimale ancora aperta [4,5]. Le fratture isolate delle tuberosità vengono classificate come fratture dell'omero prossimale a 2 frammenti secondo la classificazione di Neer e altri sistemi classificativi se presentano una scomposizione maggiore di 1 cm o una angolazione di oltre 45°; altrimenti vengono considerate come fratture a 1 frammento (composte) [6-9]. Le fratture isolate del trochite rappresentano il 14-21% delle fratture dell'omero prossimale [2,10].Tuttavia, a causa della difficoltà oggettive nel riconoscere alcuni tipi di fratture del trochite alle radiografie standard, soprattutto quelle composte o con lieve scomposizione, o con frammento piccolo che si sovrappone alla testa omerale o che può essere confuso con una calcificazione, probabilmente la percentuale reale di fratture del trochite omerale sopra riportata è sottostimata [1,4,10]. Frequentemente le fratture di trochite possono essere associate a lussazioni gleno-omerali an-23 LO SCALPELLO ( ABSTRACT -ISOLATED FRACTURES OF THE GREATER AND LESSER TUBEROSITY: DIAGNOSIS AND TREATMENTDespite the relatively common occurence of fractures of the proximal humerus in the elderly, the subgroup of isolated fractures of the greater and lesser tuberosity is still less well understood. Isolated tuberosity fractures are commonly associated with gleno-humeral dislocation or direct impact to the shoulder region. The diagnosis of greater tuberosity fracture is fairly commonly missed or ignored and this can result in an unaccettable treatment outcome. A standard radiographic trauma series of the shoulder must be obtained, including a true AP view in intrarotation and extrarotation. CT scans with coronal, sagittal and 3D reconstruction are useful for planning the surgical approach. Conservative treatment is recommended in non displaced and minimally displaced cases (5 mm in a high-demanding patient,10 mm in a low-demanding patient). The treatment of displaced fractures includes both arthroscopically assisted fixation and open reduction and internal fixation with screws or tension banding sutures. The choice of approach depends not only on fracture type but also on patient-related factors and the surgeon's experience.
