Ischiofemoral Impingement

Abstract: Ischiofemoral impingement is a syndrome defined by hip pain associated with narrowing of the space between ischial tuberosity and lesser trochanter. This phenomenon leads to abnormalities of the quadratus femoris muscle, ranging from deformity and edema to tears and atrophy. This review article presents an up-to-date discussion and imaging findings of ischiofemoral impingement, along with its related anatomy, clinical presentation, differential diagnosis and treatment.

“…This clinical scenario, termed ischiofemoral impingement, is a well-known source of posterior hip pain and is characterized by ipsilateral groin pain with ipsilateral quadratus femoris muscle edema or atrophy and narrowing of both the ischiofemoral and quadratus femoris spaces on MRI. 51,52 Edema within the iliopsoas or proximal hamstring tendons, bursa-like formations, and reduced volume of these muscles with fatty infiltration may also be seen. 51 Neuropathies can occur in any hip compartment.…”

A thorough history and physical examination focusing on the patients' presenting symptoms, medical history, and functional limitations are critical for appropriate management and prognosis.…”

“…This results in mechanical impingement of the intervening soft tissues, most frequently the quadratus femoris muscle (2,5). Both bony and soft tissue landmarks have been described to evaluate IFI.…”

“…Ischiofemoral impingement is a less commonly described source of extra-articular impingement that occurs when the space between the proximal femur/lesser trochanter and ischial tuberosity is narrowed [18,19]. These patients typically have pain with hip extension and can be developmental or secondary to prior apophyseal or myotendinous injuries at the lesser trochanter or ischial tuberosity.…”

“…These patients typically have pain with hip extension and can be developmental or secondary to prior apophyseal or myotendinous injuries at the lesser trochanter or ischial tuberosity. Magnetic resonance imaging reveals edema within the quadratus femoris muscle and relief with ultrasound or CT guided anesthetic injection into the area of impingement can be diagnostic [18]. Treatment of persistent limiting symptoms can be treated with decompression at the level of the ischial tuberosity, lesser trochanteric decompression/excision or transfer of the lesser trochanter.…”
