Ischemic priapism: Can eosinophil count and platelet functions be positive predictive factors in etiopathogenesis

Abstract: Introduction: We evaluated the relation between ischemic priapism (IP) and blood count parameters in IP patients. We especially wanted to examine the contribution of eosinophil count (EC), platelet count (PC), and mean platelet volume (MPV) values, which are suspected predictive parameters for vascular endothelium damage and venoocclusive pathogenesis and etiopathogenesis, particularly in IP. Methods: A total of 40 IP patients fulfilled the study criteria. Forty healthy volunteers in a similar age group were i… Show more

“…However, there is relatively little data on PVIs (MPV, PDW, and PCT) in priapic patients, as well as, their relationship with the outcome of primary emergency ICI of phenylephrine and the combination of aspiration and irrigation in SCA patients with acute IP (9, 10). In the study by Sönmez et al (9), the relationship between IP and high PLT and MPV was confirmed to be significant, similarly to the study by Ufuk et al (10). However, PCT was not included in the platelet parameters analyzed in this series, and statistical studies were performed with predictive and cutoff values in IP patients without SCA.…”

“…Because of the vasculopathy, chronic hemolysis, and veno-occlusive pathogenesis of SCA, multiple hematologic markers are needed to predict the outcome of IP in SCA patients. The role of platelet count (PLT) and mean platelet volume (MPV) in these veno-occlusive mechanisms is well documented (7)(8)(9)(10)(11). Previous studies confirmed the association between IP, defined as a vasculogenic disease, and platelet activation leading to higher MPV (9,10).…”

“…1 Mean platelet volume and platelet and eosinophil counts may also be elevated in men with acute ischemic priapism; while these laboratory values may contribute to the identification of underlying cause, they often will not be used to guide treatment of the acute presentation. 2 Hemoglobin electrophoresis and other sickle cell testing may be appropriate in select clinical scenarios and based on underlying risk factors (eg, patient race). In most cases, men with sickle cell disease will have been diagnosed previously.…”
