IntroductionIschemic colitis (IC) is a disease with high postoperative morbidity and mortality. Knowledge of the risk factors for postoperative mortality could be helpful in clinical decision making and in optimizing postoperative treatment.MethodsFrom a prospective database, we conducted a retrospective medical record review of 50 patients who underwent surgery for IC between 2003 and 2011 at our institution. We analyzed the causes and potential risk factors for early mortality after surgery for IC.ResultsThe early postoperative mortality and morbidity rates were 30.0% (15/50) and 54% (27/50), respectively. The two most common causes of death were multi-organ failure (66.7%, 10/15) and fulminant septic shock (20.0%, 3/15). Univariate analysis showed that postoperative mortality was significantly associated with preoperative nephropathy, coronary artery disease, a previous history of cardiovascular surgery, an ASA score ≥ 4, surgical delay ≥ 3 days, preoperative hemodynamic instability, and use of pre- and intraoperative adrenergic vasopressors. In the multivariate analysis, a previous history of cardiovascular surgery (odds ratio [OR], 8.2; 95% confidence interval [CI], 1.2–56.5) and surgical delay ≥ 3 days (OR, 5.7; 95% CI, 1.2–27.9) were identified as independent risk factors for postoperative mortality.ConclusionsBecause surgical delay is an avoidable determinant of early mortality, a high index of suspicion and early surgical intervention can increase survival. A routine postoperative evaluation for IC may be helpful in patients with a previous history of cardiovascular surgery.

La mélanose colique est une affection bénigne qui découle de l'abus de laxatifs anthraquinoniques. Elle est décrite comme une pigmentation foncée de la muqueuse colique. Elle est souvent liée à la constipation. La mélanose colique est principalement diagnostiquée par l'endoscopie et confirmée par l'histologie. Il est important de reconnaître la mélanose colique qui peut régresser généralement en quelques mois avec l'arrêt des laxatifs. Bien qu'elle soit considérée comme une maladie bénigne, les biopsies endoscopiques doivent être faites sur les zones pigmentées et les zones non pigmentées en raison de la suspicion d'adénome ou de carcinome colorectal associé. A ce propos, nous rapportons tous les cas de mélanose colique colligés au service EFD-Hépato-gastroentérologie à l'hôpital Ibn Sina de Rabat au Maroc. L'évolution était favorable après l'arrêt de l'utilisation de laxatifs chez tous les patients.

Меланоз толстой кишки характерен для пациентов с длительным анамнезом запоров и применением слабительных средств -антраценовых производных, которые довольно часто назначаются как врачами, так и пациентами самостоятельно. Как правило, меланоз развивается после 4-9 мес. приема слабительных средств этой группы и наиболее часто встречается у женщин в старшей возрастной группе. Патогенетически меланоз характеризуется отложением пигмента в собственной пластинке слизистой оболочки, проявляющимся выраженным темным окрашиванием слизистой оболочки кишки. В зависимости от степени пигментации толстой кишки меланоз подразделяют на три степени. Наиболее частая локализация -в правых отделах толстой кишки, но описаны случаи с распространением пигментации на левые и другие отделы. Диагностика не вызывает трудностей: меланоз толстой кишки выявляется при выполнении рутинного эндоскопического исследования, подтверждается при патоморфологическом исследовании. Меланоз является актуальной проблемой с точки зрения риска развития колоректального рака. Уже существуют убедительные данные, доказывающие, что частота выявления аденом повышается не только из-за отсутствия накопления ими пигмента, но и описаны патогенетические аспекты влияния меланоза на возникновение и развитие аденом. Протеомный анализ позволяет определить экспрессию различных белков, что подтверждает роль меланоза в развитии колоректального рака. Также интересно отметить, что порой существуют некоторые трудности в дифференциальной диагностике меланоза, например, с ишемическими поражениями толстой кишки. Пациенты с меланозом толстой кишки требуют пристального внимания, своевременной диагностики и лечения. Немедикаментозный метод лечения меланоза является наиболее распространенным.
