Ischemic Central Retinal Vein Occlusion and Neovascular Glaucoma as a Result of Optic Nerve Head Drusen

Abstract: Nonischemic CRVO or venous stasis retinopathy is a well-known entity associated with optic nerve head drusen. The authors report a case of ischemic CRVO in a patient with no underlying risks for this other than the observed drusen. This report clearly shows that optic nerve drusen may not be entirely benign and that they can precipitate ischemic CRVO.

“…30; 47; 58; 61; 68; 78; 108 These retinal vascular occlusions occur at a younger age than is typical for these disorders in patients without optic disc drusen; the youngest reported patient developed a branch retinal artery occlusion at the age of 11 years. 61 In most cases, patients with optic disc drusen who developed a retinal vascular occlusion at a young age also had another systemic risk factor, such as migraine, contraceptive use, systemic hypertension, atrioseptal defect, or travel to altitude.…”

“…Elevation of central retinal venous pressure may result in CRVO 1 2 6. The enlargement is observed mostly in teenage7 8 but to the best of our knowledge only a single case has been reported with concomitant CRVO in teenage 9. Unlike the hitherto reported case which resulted in neovascular glaucoma and retinal detachment, we achieved favourable outcome with intravitreal bevacizumab injection in the early treatment period.…”

“…[ 5 8 ] Although not an unprecedented etiology, ONH drusen is a rare precipitating factor. [ 2 3 9 ] The absence of conventional risk factors after an exhaustive systemic workup should prompt investigation along the lines of ONH drusen.…”
