Aims and Objectives: To find out if ischemia modified albumin can be a novel marker in ischemic heart disease. Materials and methods: Retrospective chart-review of chest pain patients who were evaluated in the emergency departments in 2006 and 2007. Results: Total of 400 patients presented with chest pain and subsequently were evaluated in the Emergency department at two major emergency departments associated with the teaching hospital associated Mercy Catholic Medical Center; few patients were excluded due to unavailability of complete data and follow-up visit in next six months. Thus, finally 351 patients met criteria for final analysis. Primary endpoints included the cardiac events and secondary outcomes were course in the hospital, days of hospitalization, interventions done in the hospital, disposition from the hospital, and readmission within next six months. Sensitivity and specificity of ischemia modified albumin (IMA) test was 80% and 50% respectively and positive predictive value and negative predictive values were 17% and 88% respectively. There were no further differences in cardiac events or interventions in higher ischemia modified albumin (IMA) group as compared to troponin (cTnT) (p=0.5). People with higher ischemia modified albumin (IMA) showed longer hospitalization days (p=0.025), needing nursing home or skilled nursing facility on discharge due to high discharge needs (p=0.0001) and had more readmissions as compared to troponin (p=0.02). However, Higher ischemia modified albumin (IMA) did not predict more cardiovascular events during hospital stay, rather troponin (cTnT) test predicted arrhythmia more than ischemia modified albumin (IMA) test (p=0.0001). Conclusion: Ischemia-modified albumin may be utilized as a novel marker of ischemia to rule out acute coronary syndrome along with troponin and electrocardiogram in the emergency departments. This test may also have short and long-term prognostic significances and may even be used as a surrogate marker in stroke, sepsis and thrombo-embolic diseases once these people need admission to the hospital.

Background: Ischemia-modified albumin (IMA) is an altered type of serum albumin that forms under conditions of oxidative stress and an independent predictor of major adverse cardiovascular events. Objectives: To measure the levels of IMA in 45 children and adolescents with β-thalassemia major (β-TM) compared with 30 healthy controls and assess its relation to lipid peroxidation, vascular complications and subclinical atherosclerosis. Methods: β-TM patients without symptoms of heart disease were studied focusing on transfusion history, chelation therapy, serum ferritin, malondialdehyde (MDA) and IMA levels. Echocardiography was performed and carotid intima media thickness (CIMT) was assessed. Results: IMA and MDA levels were significantly higher in β-TM patients compared with controls (p < 0.001). IMA was higher among patients with heart disease, pulmonary hypertension risk and serum ferritin ≥2500 µg/l than those without. TM patients compliant to chelation had significantly lower IMA levels. IMA levels were positively correlated to MDA and CIMT while negatively correlated to ejection fraction and fractional shortening. Conclusion: Our results highlight the role of oxidative stress in the pathophysiology of vascular complications in thalassemia. IMA could be useful for screening of β-TM patients at risk of cardiopulmonary complications and atherosclerosis because its alteration occurs in early subclinical disease.

Concentraciones de albúmina modificada por la isquemia en niños con asma: estudio piloto INTRODUCCIÓNSe define asma como la repetición de síntomas respiratorios como sibilancias, falta de aire, opresión en el pecho y tos, que varían con el tiempo y en intensidad, junto con una limitación variable del flujo de aire en la exhalación. La exacerbación del asma se caracteriza por la disminución progresiva de la función pulmonar y e l a u m e n t o p a u l a t i n o d e l o s síntomas. La característica principal de una exacerbación del asma es la obstrucción del flujo de aire en las vías respiratorias. Para determinar la gravedad de la exacerbación del asma, la hipoxia que provoca la obstrucción de las vías respiratorias se evalúa midiendo la saturación de oxígeno. El objetivo del tratamiento es contrarrestar rápidamente la obstrucción del flujo de aire y la hipoxemia, abordar la patofisiología inflamatoria subyacente y evitar las recaídas. La primera medida para tratar la exacerbación del asma en el departamento de emergencias, es administrar oxígeno mediante una mascarilla para lograr y mantener la saturación percutánea de oxígeno entre el 94% y el 98%. 1 E l e x t r e m o t e r m i n a l d e l aminoácido en la albúmina tiene la capacidad de unirse a metales pesados (cobre, níquel y cobalto). En condiciones patológicas como la hipoxia o la acidosis, el daño al radical libre que tiene lugar en caso de isquemia reduce la capacidad de unión de los metales al dominio N-terminal de la albúmina.
