Is unilateral tonsillar enlargement alone an indication for tonsillectomy?

Abstract: The prevalence of malignancy in tonsils which exhibit asymmetry with no other clinical features is very low; in our study it was zero. However, other studies have found a small percentage representing underlying malignancy. In view of this, we feel that a 'watch and wait' policy is initially more appropriate, and if symptoms or signs are progressive, tonsillectomy should then be advised.

“…In this context, the factor of obesity seems to be of high relevance that may reduce the success rate from 70% to 80% to 10% to 25% [97], [130]. Based on this guideline, tonsillar asymmetry is not considered as indication of TE, except for suspected malignoma [460], [461], [462]. …”

“…1 The index of suspicion of a malignant process should be high in older age groups, especially if there is a history of tobacco and alcohol consumption or a past history of malignancy. 2,3 There is debate over whether a 'watch and wait' approach can be adopted for unilateral tonsillar enlargement to avoid unnecessary surgery and its complications. In our case, proceeding to a tonsillectomy without imaging could have led to multiple complications owing to the displacement of the tonsil, as can be seen in the CT (Fig 2).…”
