Is there a relationship between systemic perfusion temperature during coronary artery bypass grafting and extent of intraoperative ischemic central nervous system injury?

Abstract: In this series of 291 patients randomized to perfusion temperature, we observed no relationship between the size of a cerebral ischemic infarct and the perfusate temperature during coronary revascularization.

“…In fact, controversial results have been reported regarding the use of hypothermic strategies for myocardial (11,(38)(39)(40)(41)(42)(43)(44) and cerebral protection (38,40,(45)(46)(47)(48)(49) during adult cardiac surgery. Up to now, no study provided conclusive results on the beneficial or harmful effects of mild/moderate hypothermia during CPB.…”

“…Certain maneuvers can distort their measurement; for instance, high levels of S100␤ are present in the blood drained from the mediastinum by cardiotomy suction reflecting the presence of the protein in mediastinal fat cells. 81 Factors such as hemodilution, hemolysis, extracerebral liberation, cardiotomy suction use, 48 auto transfusion or renal failure may be confounding factors regarding biochemical evaluation of neuronal damage. 7,24,88 Recently, Siman et al 100 evaluated variations in 5 potential CSF markers for neurologic damage in patients undergoing aortic aneurism repair with or without DHCA.…”

“…Indeed, the definition of warm CPB is quite different in different studies, ranging from 33 to 37 °C. More recently, the use of what is called ‘tepid’ bypass temperatures (31–34 °C) has appeared in the literature [1, 9, 10]. Only studies of patients undergoing coronary artery bypass grafting are listed in Table 3.…”
