Is the All-arthroscopic Tibial Inlay Double-bundle PCL Reconstruction a Viable Option in Multiligament Knee Injuries?

Abstract: Background All-arthroscopic tibial inlay double-bundle (DB) posterior cruciate ligament (PCL) reconstruction avoids an open dissection and the ''killer turn'' while maintaining the advantage of an anatomic graft. However, clinical data on the viability of this surgical technique in multiligamentous knee injuries are lacking. Questions/purposes At greater than 2 years of followup, we evaluated (1) validated outcomes scores; (2) range of motion; and (3) side-to-side stability on PCL stress radiographs of a small… Show more

“…Mean subjective IKDC score, however, was above the threshold for acceptable symptom status (62 ± 14.4 according to Muller et al [20]). These scores were also similar to those in other studies, especially for associated PCL lesion, such as that by Weber et al [21]. The present results thus provided satisfactory knee function in daily life.…”

“…Steeper GBAs (as high as 90°) occurred in certain patients, similar to the ''killer turn'' referred to for posterior cruciate ligament reconstruction, which has been associated with friction, graft stretch, and fatigue failure. 54,56 A similar graft deformation to our example of a steep GBA with ACL dent formation ( Figure 5) has been reported at the femoral aperture corner by applying cyclic loads to cadaveric knees after anatomic ACL reconstruction. 31 Our patient with a reinjury demonstrated a mean GBA of 87.1°and 91.1°during walking and running, respectively.…”

“…Le score IKDC subjectif moyen était néanmoins supérieur à celui habituellement considéré comme non acceptable sur le plan fonctionnel et estimé à 62 ± 14,4 par Muller et al [20]. Ces scores étaient également similaires à ceux d'autres études notamment en cas de lésion associée du ligament croisé postérieur, comme le travail de Weber et al [21]. Nos résultats permettaient donc d'obtenir un genou satisfaisant dans la vie courante.…”
