Is the 6‐item, self‐report HAM‐D an effective depression screening measure with bipolar disorder?

Abstract: Brief and effective screening measures are required to detect clinical depression in bipolar disorder (BD) patients. The purpose of this study was to demonstrate the clinical utility of a six-item, self-report Hamilton Depression Rating Scale (HAMD-6). Design and Method: Sixty BD outpatients completed questionnaires including the HAMD-6 before regular psychiatric appointments. Findings: A 7+ HAMD-6 cutoff score correctly identified six of eight depressed patients, indicating 75% sensitivity and 84% specificity… Show more

“…Consistent with existing research [ 47 , 48 ], depression and hypo/mania are positively correlated (r=.29; P= .01), suggesting that depression and hypo/mania are not inverse clinical states. Often participants reported both types of BD symptoms (eg, mixed features).…”

“…The BDS x was developed for ecological momentary sampling of BD symptoms via smartphone app [ 43 , 44 ] but has also been validated for use online [ 45 , 46 ] and as a printed-page screening measure with BD outpatients [ 47 , 48 ]. In this study, α=.88 for depressive symptoms and α=.71 for the hypo/mania subscale.…”

“…Concurrent validity of BDS x responses by BD I outpatients has been demonstrated relative to the self-reported Hamilton Rating Scale for Depression (HAMD-6) and the Altman Self-Rating Mania Scale (Kraun et al 2020). Similarly, sensitivity (and specificity) are high for the BDS x depression subscale at 88% (and 76%), respectively (Osher et al 2020).…”

“…Internal consistency of BDS x responses by BD I outpatients was recorded as α = 0.90 for the depression subscale (cognitive + somatic symptoms; Osher et al 2020) but lower for the hypo/mania subscale at α = 0.76 (affrontive + elation/loss of insight; Kraun et al 2020). This difference may be due to lower frequency of hypo/manic versus depressive symptoms (Judd et al 2002).…”

“…The HAM-D on review, assesses severity of depression and should not be used for comparison to the DSM-5 diagnostic criteria for major depression. Current versions of the HAM-D demonstrated a sensitivity of 75% and a specificity of 84% for depression screening and take about 20-30 minutes to administer (Kraun et al, 2020). This instrument demonstrates good validity and utility for assessing depression severity but not for diagnosis of mood disorders (Kraun et al, 2020).…”
