Is persistent polyclonal b lymphocytosis caused by epstein‐barr virus? A study with polymerase chain reaction and in situ hybridization

Abstract: Previous reports of patients with persistent polyclonal B lymphocytosis have found associations with female sex, cigarette smoking, HLA-DR7 phenotype, and moderate elevation of peripheral blood polyclonal B lymphocytes. The presence of binucleated cells and atypical lymphocytes in the peripheral blood of these patients was highly suggestive of a viral infection, such as with the Epstein-Barr virus. We report a 47-year-old asymptomatic woman who was incidentally found to have persistent polyclonal B lymphocytos… Show more

“…25 Similarly, a specific fragment of the LMP-1 gene was present in all seven PPBL patients but undetected in PBMC of normal subjects. Our recent data and others [16][17][18] strongly support the presence of a latent EBV infection in patients with PPBL. HLA-DR7, present in most PPBL patients, might potentially play a role in facilitating viral infection.…”

“…Because EBV has also been identified in cells of patients with PPBL, 16,17 we hypothesized that our patient might have an EBV-related lymphoma. Several genes are believed to play a critical role for the immortalizing function of EBV; of these, EBNA-1 is essential for viral replication and always expressed in NHL associated with EBV.…”

“…Evidence of past EBV infection is more common than in the general population, and Chow et al (1992) demonstrated the presence of EBV DNA in the abnormal lymphocytes of an affected patient. The association with cigarette smoking is equally strong, and in one reported case the lymphocytosis resolved on stopping and returned on restarting smoking (Carstairs et al, 1985).…”

“…A non-isotopic method, using a mixture of fluorescein isothiocyanate (FITC)-labelled IL-6 antisense oligonucleotides, was used to determine the expression of IL-6 mRNA in cell lines and pathological sections (Chow et al, 1992). The probe sequences were (Wang et al, 1988): 5′-GGACAGGTTTCTGACCAGA-AGAA GGAATGCCC-3′ (IL-6 mRNA, 750-719) 5′-ACT-GCAGGAACTCCTTAAAGCTGCGCAGAA-3′ (IL-6 mRNA, 660-631) 5′-TCACCAGGCAAGTCTCCTCATTGAATCCAG-3′ (IL-6 mRNA, 390-361).…”
