Is peritoneal dialysis causing a measurable burden of inflammatory and endothelial injury on top of metabolic syndrome?

Abstract: In patients on PD, inflammatory reaction is higher than in patients with MS. On the contrary, patients with MS have stronger oxidative stress response and adipose tissue activity caused probably by the chronic low level of inflammation and underlying metabolic disorders.

“…The elevation of CRP and proinflammatory cytokines, such as IL‐6, due to the interaction between blood and dialyzer membrane is an important contributing factor in HD patients . Some recent studies report that in PD patients, endothelial dysfunction and oxidative stress cause subtle chronic inflammation . Interestingly, chronic inflammation leads to higher occurrence of NODM in PD patients .…”

“…Patients with MS who are receiving PD always have low-grade, chronic inflammation. 21,22 Our findings may indicate a stronger inflammatory reaction in patients with MS than in those without MS. MS has been reported as an independent risk marker for CVD morbidity and mortality in patients without DM who are undergoing PD. 1,23 Patients with MS had a higher risk of arteriosclerosis incidence than those without MS.…”

“…Malnutrition may affect the normal function of the vascular endothelium, leading to abnormal regulation of vasodilation and constriction, thereby increasing the risk of cardiovascular events. [36,37] Notably, although no significant association of the GNRI with technique failure or peritonitis was observed, only 2 included studies performed relevant explorations. Therefore, it is still necessary to identify the predictive role of the GNRI for technique failure and peritonitis among PD patients in future research.…”
