Is jaw functional orthopedics an efficient tool to treat anterior open bite? A retrospective study

The vertical problems of the stomatognathic system that alter the overbite, either by increasing in the deep bite (DB) or decreasing in the anterior open bite (AOB), are among the great challenges of the dentofacial corrections in the treatment and the retention and stability protocols. It is always good to report that a correct diagnosis is fundamental in the choice of treatment protocol and that skeletal involvement, be it hyper divergence in case of open bite or hypo divergence in case of deep bite, aggravates the problem and as a rule tends to increase treatment and containment time. In this paper the state of art of AOB treatment with Jaw Functional Orthopedics (JFO) will be discussed. It should be emphasized that JFO has a unique diagnostic tool for changes in vertical growth of the face, the Articular Compass, developed by Simões. Studies show that the use of treatment protocols developed by Simões, the SNs (SN2, SN3, SN6) act in the increase of overbite efficiently (P= 0.003) in cases of open bite. This is weak evidence, due to the design of the study, but with promising results, so Randomized Clinical Trials are needed to investigate if these results are replicable.

Anterior Open Bite (AOB) is a type of malocclusion that promotes severe esthetic aesthetic and functional impairment. Its etiology is multifactorial and may involve several etiological factors, such as deleterious oral habits, hypertrophic tonsils, mouth breathing, dental ankylosis and eruption abnormalities either associated with genetic factors or not. AOB is one of the most challenging treatments in any treatment protocol. A retrospective study of 21 patients with anterior open bite treated with Jaw Functional Orthopedics (JFO) with improvement in overbite was conducted with the aim of understanding mandibular behavior during the treatment. Using SPSS 11.0, using Shapiro-Wilk test all variables showed Normal distribution (p> 0.05), in the comparison between the means at T0 and T1, the t-test for paired samples was used. In order to verify whether cooperative patients had better results than the non-cooperative group, a variable was created. For all tests, a significance level of 5 % was used. A weak statistical correlation (P< 0,1) was found for Bimler’s Factor 8. Change resulted from AOB treatment with FOA in a short time. There was no statistical correlation of Bimler’s Factor 8 when cooperative patients were compared with the non-cooperative group. It could be concluded that in the studied sample there was a weak correlation between overbite improvement during AOB treatment with JFO and mandibular rotation. Further studies are necessary to understand the skeletal behavior of the maxillary bones in the treatment of AOB with FOA.

Use of functional orthopedic appliances (FOA) in the treatment of malocclusion and Temporomandibular Disorders (TMD) has been proved to be effective but there is still questions to be answered like the muscular action of the referred appliances. The aim of this study is checking through a proven protocol of surface electromyography (sEMG) to study muscular action of FOA to check to check if it is correctly manufactured. The appliance studied is a Simões Network 10 – SN10 to treat Class II malocclusion of retrognathia. The sEMG was collected 1 patients with class II malocclusion with retrognathia who belong to a 164 volunteers with malocclusion, in two times T1 before installation of the FOA in mouth, T2 15 minutes after the FOA installation in the mouth. sEMG data of bilateral masseter, bilateral temporal and bilateral suprahyoid muscles using conditioner signals module from Lynx Electronics Ltda with 8 channels, model EMG1000; software AqDAnalysis 4,18 from Lynx Electronics Ltda.; Software Lynx BioInspector 1,8r; passive surface electrodes (Ag/AgCl) from Noraxon Dual Electrodes (USA); dischargeable reference electrodes Kendall Meditrace (Ag/AgCl) – Canada were used for the sEMG measurements. Frequency calibration was 2000 Hz, with 2048 sample by channel and time 1,024 seconds, and filters regulation was 20 Hz and 1000 Hz. With the FOA in the mouth all measurements improved with a more simetrical sEMG in T2 in rest and isometric contraction measurements. The protocol used to check the manufacturing of functional orthopedic appliances using surface EMG proved to be a valid tool in this case report study. Further investigations are needed to confirm this protocol and check if the same happens with other types of functional orthopedics appliances.
