Is It Ovarian Torsion? A Systematic Literature Review and Evaluation of Prediction Signs

Abstract: Abdominal pain in children and adolescents is difficult to evaluate, and the diagnosis of ovarian torsion remains a challenge. Because of its potential complications, we need effective clinical tools. From our review of the literature, it was not possible to develop a diagnostic algorithm. Further research is needed to improve our practice and shorten the delay to diagnosis. Considering the low incidence of ovarian torsion, a multicenter prospective study would be required.

“…The significantly enlarged right ovary and abnormal spectral arterial waveforms were highly suspicious for torsion which was confirmed by laparoscopy. Aligning with our patient's case, a recent systematic review of ovarian torsion in the pediatric population identified right lower abdomen as the most common location of pain [11]. It has been suggested that right-sided torsion predominance may be due to the ample space surrounding the right ovary in comparison to the left ovary which is confined by the occupation of the sigmoid colon [12].…”

“…Ovarian torsion due to an adnexal mass causes various symptoms and signs on clinical presentation. The most common symptom is acute onset of lower abdominal pain, followed by nausea and vomiting [ 15 22 24 33 34 35 ]. Some patients experience waves of nausea with or without vomiting [ 15 22 24 33 ].…”

“…Ovarian torsion (adnexal torsion or turbo-ovarian torsion) is a cause of acute abdominal pain in women, which requires emergency gynecological surgery [ 1 ]. Although adnexal torsion is most common in sexually mature women, it can also affect pre-pubertal and post-menopausal women [ 1 ]. Early diagnosis and treatment are important to salvage the viability of the ovary and to maintain fertility [ 1 3 ].…”
