Is it better to use the radial artery as a composite graft? Clinical and angiographic results of aorto-coronary versus Y-graft1

Abstract: The RA provides the same clinical and angiographic results both as aorto-coronary and composite Y-graft with the left ITA. When the RA is used as Y-graft the procedure is more technically demanding and a greater number of distal coronary anastomoses is possible. RA grafts to targets with stenosis < or = 70% appear to be at risk of failure.

“…Previous angiographic observational studies have shown that the RA achieved excellent short-(96% -100%), mid-(94% -97%), and long-term graft patency (84% -96%) when used as either an aortocoronary bypass or a composite graft [13,[29][30][31]. reported similar long-term graft patency [44][45][46][47].…”

“…The use of the bilateral ITAs was also proposed and a better clinical evolution was confirmed by some authors [7][8][9] and contested by others [10,11]. It is frequently not used due to the increase in morbidities and the complexity of the technique [7,12].…”

“…The RA is easy to dissect, has a good caliber and is long enough to reach the arteries of the posterior face of the heart, and can be used in sequential anastomoses [7,13]. Its negative points are: the presence of atheroma plaque in elderly patients and a greater tendency for vasospasms due Table 6.…”

“…The use of artificial T-graft or Y-graft, as well as sequential anastomoses, can maximize the usage of arteries, theoretically expanding the long-term benefit [23,24]. In spite of encouraging evidences there is still a lack of evidences to support the routine use of these techniques.…”
