Is Additional Systematic Biopsy Necessary in All Initial Prostate Biopsy Patients With Abnormal MRI?

Abstract: PurposeTo determine whether additional systematic biopsy is necessary in all biopsy naïve patients with MRI visible lesions by taking PI-RADS score and prostate volume into consideration.Materials and MethodsPatients who underwent combined systematic biopsy (SB) and cognitive MRI-targeted biopsy (TB) in our hospital between May 2018 and June 2020 were retrospectively reviewed. The detection rate of clinical significant prostate cancer (csPCa), biopsy grade group (GG) concordance, and disease upgrading rate on … Show more

“…Additionally, the CDR of PI-RADS5 is very high (86–88%). 29 30 In patients with had index lesion of PI-RADS 5, the CDR in target biopsy is very high; therefore, the significance of systematic biopsy might be reduced. Moreover, in patients with PI-RADS 3 index lesions, the significance of systematic biopsy would also be reduced, because the overall CDR is low inpatients with PI-RADS 3 index lesion.…”

“…At present, transrectal ultrasound (TRUS)-guided core biopsy is the most frequently performed procedure [6]. However, the tumour location can be detected only in a small number of cases (≤ 30%) when TRUS is applied [7].…”

“…It is often difficult to obtain cancer tissue. This leads to a large percentage of false-negative results, which is related to consecutive biopsies and treatment [6,8].…”

“…MRI‐based grading (5‐point scoring system), such as the Prostate Imaging Reporting and Data System (PI‐RADS) or Likert scores, plays a major role in stratifying the superiority of targeted biopsy over SB 9,12–15 . Specifically, MRTB has shown to perform better with a greater PI‐RADS score than SB 13,14,16–18 …”

“…9,[12][13][14][15] Specifically, MRTB has shown to perform better with a greater PI-RADS score than SB. 13,14,[16][17][18] Radiomics is a promising approach that facilitates highthroughput extraction of image features from radiographic images to characterize the underlying tumor microarchitecture and heterogeneity, which can be subtle and may not be immediately visible on routine imaging. 19 It is an important adjuvant tool of PCa detection and risk stratification.…”
