Is a national framework for implementing early psychosis services necessary? Results of a survey of Australian mental health service directors

Abstract: The disturbingly high levels of variability in EPI implementation across jurisdictions suggest a pressing need for a set of nationally agreed uniform EPI implementation standards. Additional specific strategies for rural and remote mental health services may be needed for these services to implement EPI.

“…Consistent with other studies (Catts et al, 2010;Lester et al, 2009), the lack of funding and staff are also considered important barriers to the development of EI services. This problem seems to be present on a worldwide scale, as even in Australia, which was one of the first countries to start a reform towards EI, implementation has been affected by insufficient allocation of resources to appropriately support the reform focus (Catts et al, 2010).…”

In this regard, it should be also noted that the Italian EI services are being mostly developed according to a "specialist within generalist team" model (Cocchi et al, 2011a;Ghio et al, 2012), mainly due to lack of funding and in contrast with other experiences that favor the implementation of specific and independent outpatient services (Edwards et al, 2002).…”

“…It reports on the struggles that rural people have in identifying emergent mental health problems and in selecting suitable professional help to address these problems. It is well known that early identifi cation and intervention of mental health problems has a signifi cant impact on the extent of disability and the prognosis trajectory (Amminger et al, 2006;Berk et al, 2007;Catts et al, 2010;Edwards & McGorry, 2002;EPPIC, 1998;Henry et al, 2007;McGorry, 2004;McGorry, Killackey, Yung, 2007). The earlier that interventions are applied, the better the prognosis, while conversely, long durations of untreated illness adversely infl uences recovery (McGorry, 2004;McGorry, Killackey, Yung, 2007).…”

“…The strongest evidence for interventions to be included in guidelines usually stems from controlled or -even better -randomised controlled trials. Common outcomes of these trials include symptom reduction, improvement of daily functioning and use of healthcare services, mainly hospital services (Catts et al 2010;Stegbauer et al 2015). Outcome measures related to patient experiences or priorities are underrepresented in studies that evaluate the effectiveness of (mental) healthcare interventions.…”
