“…5 Much of this increased surgery must be unnecessary, and high surgical rates have been reported from the UK, Western Europe, Scandinavia, North America, Latin America and South Africa. The association of IBS with normal appendiceal pathology after urgent appendectomy, as reported by investigators from Taiwan in this issue, 9 (see page 655) further emphasises the worldwide importance of avoiding unnecessary surgery in IBS.…”

“…Furthermore, IBS subjects usually receive additional abdominal surgeries. It is likely due to their coexisting multiple somatic and psychiatric comorbidities leading to the unexpected surgeries 31,32. Very similarly, migraineurs also manifest these overlapped comorbidities including IBS 8,9,10,33,34…”

“…However, the therapeutic dilemma centres on the patient with persistent symptoms in whom all investigations are completely normal, where the most likely diagnosis is a functional gastrointestinal disorder, particularly irritable bowel syndrome (IBS). Many patients with the more severe forms of IBS give a history of multiple surgical interventions, including appendicectomy, and seem to be particularly susceptible to postoperative pain syndromes [Longstreth and Yao, 2004;Cole et al 2005;Lu et al 2007]. Consequently, it currently seems reasonable to conclude that in patients with investigation-negative right lower quadrant abdominal pain, with additional symptoms suggestive of a functional gastrointestinal disorder, surgical intervention is best avoided.…”
