Therapeutic Effects of Lactobacillus in Treating Irritable Bowel Syndrome: A Meta-analysis

Objective As the lack of reliable treatment for irritable bowel syndrome (IBS) prompts interest in the development of new therapies, we aimed to systematically evaluate the effect of Lactobacillus in treating this disease. Methods We searched MEDLIINE, PubMed, Scopus, Web of Science and the Cochrane Central Register of Controlled Trials for the period from 1966 to August 2013 for double-blind, placebo-controlled trials investigating the efficacy of Lactobacillus treatment in the management of IBS. The studies were screened for inclusion based on randomization, controls and reported measurable outcomes. We used the Jadad score to assess the quality of the articles. The STATA 11.0 and Revman 5.0 software packages were used for the metaanalysis. The STATA 11.0 software program was also used to assess indicators of publication bias according to Begg's and Egger's tests. Results Six randomized, placebo-controlled clinical trials met the criteria and were included in the metaanalysis. The Jadad score of the articles was >3, and three articles were of high quality. We analyzed the heterogeneity of the studies and found no heterogeneity in the meta-analysis. In the forest plot, the diamond was on the right side of the vertical line and did not intersect with the line. The pooled relative risk for clinical improvement with Lactobacillus treatment was 7.69 (95% confidence interval: 2.33-25.43, p=0.0008). For adults, the pooled relative risk for clinical improvement with Lactobacillus treatment was 17.62 (95% confidence interval: 5.12-60.65, p<0.00001). For children, the pooled relative risk for clinical improvement with Lactobacillus treatment was 3.71 (95% confidence interval:1.05-13.11, p=0.04). Using the STATA 10.0 and Revman 5.0 software programs, we confirmed that Lactobacillus exhibited significant efficacy in treating IBS. Conclusion Compared with the placebo, Lactobacillus treatment was found to be associated with a significantly higher rate of treatment responders in the overall population with IBS, without any side effects. As to limitations of the analysis, additional research is needed.

Latar belakang. Irritable bowel syndrome (IBS) merupakan nyeri perut berulang pada remaja yang paling banyak terjadi. Oleh karena itu, IBS akan menimbulkan gangguan perilaku yang serius pada remaja. Tujuan. Menganalisis hubungan antara masalah perilaku dengan IBS pada remaja. Metode. Penelitian potong lintang semua siswa SMA Nurul Iman. Pencatatan dilakukan meliputi karakteristik umum dan pemeriksaan fisis. Selanjutnya dilakukan uji Rome III (irritable bowel syndrome) dan PSC-17 (masalah perilaku). Analisis statistik digunakan analisis bivariat dengan uji chi-square. Hasil. Dari siswa SMA Nurul Iman didapatkan 180 subyek yang memenuhi kriteria inklusi dan tidak memenuhi kriteria eksklusi. Prevalensi IBS 58 (32,2%) subyek, terdiri atas 22 subtipe konstipasi, 23 subtipe diare, dan 13 subtipe campuran. Prevalensi masalah perilaku 40,6%, terdiri atas 28,9% masalah perilaku internalisasi, 2,8% masalah eksternalisasi, 0,6% masalah perilaku perhatian, dan 8,4% variasi dari 3 gangguan. Faktor risiko terjadinya IBS, antara lain, mengonsumsi daging olahan, teh, makan terburu-buru, serta di-bully. Terdapat hubungan yang bermakna antara IBS dengan masalah perilaku (p=0,001). Nilai Odds Ratio yang diberikan 3,015 (IK95%=1,580-5,754) Kesimpulan. Remaja yang mengalami IBS mempunyai risiko yang meningkat untuk terjadinya masalah perilaku. Irritable bowel syndrome (IBS) is the most common recurrent abdominal pain in adolescence, causing serious impairments on behavioral problems. Objective. To assess for an association between IBS and behavioral problems in adolescences. Methods. Subjects in this cross-sectional study were adolescences who attended Nurul Iman high school. Their general characteristics and physical examination results were recorded. We administered the Criteria Rome III for IBS and the Pediatric Symptom Checklist 17 (PSC 17) for behavioral problems. Data was analyzed by Chi-square test. Results. We enrolled 180 adolescences as student in Nurul Iman high school. Prevalences of IBS was 32,2%, consisting of subtype IBS constipation (37,9%), subtype IBS Diarrhea (39,7%), and subtype IBS Mixed (22,4%). The prevalence of behavioral disorders was 40,6%, consisting of internalization (28,9%), externalization (2,8%), attentive problems (0,6%) and various combinations of three problems (8,4%). A significant association was found between IBS and behavioral problems (P=0.001; PR=3.015; 95%CI=1.580-5.754). Conclusion. IBS is significantly associated with behavioral problems. Irritable bowel syndrome (IBS) in man is not a single entity but has several causes. One of the most common forms has similarities with colic and laminitis in horses. Undigested food residues may pass from the small intestine into the colon where bacterial fermentation produces chemicals that lead to disease. In horses the consequences may be disastrous, but in healthy humans such malabsorption may not be harmful. After events such as bacterial gastroenteritis or antibiotic treatment, an imbalance of the colonic microflora with overgrowth of facultative anaerobes may arise, leading to malfermentation and IBS. It is not known whether such subtle changes may likewise be present in the microflora of horses who are susceptible to colic and laminitis. Metabolomic studies of urine and faeces may provide a suitable way forward to identify such changes in the horse's gut and thus help to identify more accurately those at risk and to provide opportunities for the development of improved treatment.
