Irreducible fracture-dislocation of the knee

Abstract: Dislocation and fracture-dislocation of the knee are serious injuries, often related to high-energy trauma. Irreducibility with closed techniques is an extremely rare and challenging problem that has been described in posterolateral, posterior and lateral knee dislocations. Irreducibility in fracture-dislocations around the knee has only been described twice in the literature and never in association with a tibial plateau fracture. We report a unique case of an irreducible tibial plateau fracture-dislocation i… Show more

“…Interestingly, after fracture fixation, the joint was stable in physical examination (negative anterior/ posterior drawer test, negative varus/ valgus stress test) and no gross ligament injury was detected. There is a case report in the literature describing a posteromedial knee fracture dislocation scenario with no knee ligament injury [ 12 ]. We think our patient’s associated articular fractures may protected knee ligaments from severe damage.…”

“…After 1 month, the patient underwent another operation in order Figure 5B. X-ray: LL view the surgical repair and then bone stabilization (14) The second factor is the association between knee dislocation and open fractures (15). The third factor is represented by irreducible dislocation of the knee (16,17).…”

“…9 In the third case, the reduction of the knee was not possible due to the incarceration of the medial meniscus within medial tibial plateau fracture. 10 revealed the right knee held in a slight flexion by 30 degrees and valgus deviation of 15 degrees. There was a diffuse tenderness around the knee with a transverse ecchymotic "puckering" of the skin over the anteromedial thigh; the medial femoral condyle was palpable just above the "pucker" (►Fig.…”
