IOIBD technical review on endoscopic indices for Crohn's disease clinical trials

Abstract: At the end of the voting process, the investigators ranked first a >50% decrease in Simple Endoscopic Score for Crohn's Disease (SES-CD) or Crohn's Disease Endoscopic Index of Severity for the definition of endoscopic response, and an SES-CD 0-2 for the definition of endoscopic remission in CD. All experts agreed on a Rutgeerts' score i0-i1 for the definition of endoscopic remission after surgery.

“…(16) Validated endoscopic scoring systems include the CD Endoscopic Index of Severity (CDEIS) and the Simple Endoscopic Score for CD (SES-CD). (4, 43, 44) The CDEIS consists of recording 9 descriptors across 5 segments including the ileum. The weaknesses of the CDEIS include its complex requirement of data recording, possible underestimation of the severity of disease when only one out of five segments is involved, and the absence of validated score thresholds associated with specific prognostic outcomes.…”

“…Regarding CD, a panel of experts recently proposed the threshold for endoscopic remission to be defined as a SES-CD score (Simplified Endoscopic Score for Crohn's Disease) of between 0 and 2 [8]. In current practice, Rutgeert's score is used to evaluate the endoscopic postoperative recurrence commonly defined as a score ≥ i2 (5 or more aphtous ulcers at the site of anastomosis).…”
