Iodine-Induced Hypothyroidism in Full-term Infants With Congenital Heart Disease: More Common Than Currently Appreciated?

Abstract: Iodine-induced hypothyroidism may be under-recognized in infants with CHD exposed to excess iodine. Systematic monitoring of thyroid function should be considered to avoid potential long-term adverse neurodevelopmental effects of even transient thyroid dysfunction in this susceptible population.

“…This can occur especially in premature babies (28,29) and in term infants with congenital heart disease needing cardiac catheterization (30,31). Preterm infants get exposed high doses of iodine with usage of iodine containing antiseptics for umbilical and peripherally inserted central catheter (PICC) lines.…”

“…Infants may be particularly vulnerable to iodine deficiency and iodine excess as the fetal and newborn thyroid has limited iodine stores and adapts poorly to high intakes (3,4,5). Acute iodine excess from for example maternal iodine supplements and iodine containing skin disinfectants may cause hypothyroidism in newborns (6,7). Recent data indicate that older infants may be able to adapt to high iodine intakes and maintain their euthyroid state (8).…”

“…15 Furthermore, iodine in topical antiseptic agents and contrast media can affect thyroid function of preterm neonates. 16 In our NICU, iodine antiseptic agent has not been used for many years, so any effect of topical iodine antiseptic agents could be ruled out. Additionally, those medications that might affect thyroid function were not administered to the three twins.…”
