Investigational Research: Timeline, Trials, and Future Directions of Spinal Disc Arthroplasty

Abstract: Spinal disc arthroplasty (SDA) has been a rising alternative to fusion for the treatment of degenerative disc disease (DDD). This review aims to provide an overview of the timeline, approvals, and limitations of SDA through analysis of U.S. Food and Drug Administration (FDA)-approved trials. Clinical studies have shown more successful outcomes when comparing cervical disc arthroplasty (CDA) with anterior cervical decompression and fusion, with the key benefits of decreased risk of nerve root compression and ad… Show more

“…12 After the publication of this landmark study, the first CDA was approved by the Food and Drug Administration (FDA) later in 2007. 13 Since then, multiple randomized controlled trials and meta-analyses have been performed with positive 2,3,[14][15][16][17][18] as well as cautionary 2,16,[19][20][21] results. Some have raised concerns about possible bias in the initial randomized controlled trials comparing CDA and ACDF.…”

“…The first randomized controlled trial to compare CDA to ACDF was published in March 2007 by Mummaneni et al 12 This study found that CDA resulted in improved disability, pain, and return to work along with reduced revision and reoperation rates when compared with ACDF 12 . After the publication of this landmark study, the first CDA was approved by the Food and Drug Administration (FDA) later in 2007 13 . Since then, multiple randomized controlled trials and meta-analyses have been performed with positive 2,3,14–18 as well as cautionary 2,16,19–21 results.…”

“…Six of these devices have been approved for 1-level use, whereas the remaining 3 to have approval for 1-and 2-level use (Mobi-C, Prestige-LP, Simplify). 13,14 There are currently no FDA-approved indications for greater than 2 levels of CDA. Current indications include 1 or 2 contiguous levels of cervical pathology; herniated cervical disc without significant loss of disc height; cervical spondylosis or degenerative disc disease; and cervical radiculopathy or myelopathy.…”
