Objectives. This study is aimed at investigating the root and root canal morphology by cone beam computed tomography (CBCT) and palatal furcation groove of the buccal root by microcomputed tomography (micro-CT) of maxillary first premolars in a Chinese subpopulation. Methods. This study assessed CBCT images of 440 patients aged 14-80 years. Based on Vertucci’s classification, the number of roots and the canal configuration were determined. Forty-eight maxillary first premolars with furcation grooves were analyzed by micro-CT in patients aged 18-25 years. Results. Based on the CBCT assay, 70.22% and 29.32% of maxillary first premolars were 1 root and 2 roots, respectively. The configuration indicated statistical difference ( P < 0.05 ) between male and female patients. The most common canal type was type IV and was found in 44.32% of cases, followed by type I in 27.84%, and then type II in 20.57%. Root bifurcations had 40.13% prevalence which was distributed more in the middle third than in the cervical and the apical third. For the micro-CT study, 95.83% of the furcation groove configuration was found in the bifurcated maxillary first premolars. The length varied from 1.02 to 7.63 mm. The mean depth of this groove was 0.57 mm in the root coronal, 0.47 mm in the root middle, and 0.22 mm in the root apical level. Palatal dentin width was smaller than 1 mm. Conclusion. The anatomy of the root and root canal system and the irregular wall width of maxillary first premolars with furcation grooves may help dentists to understand the anatomical morphology and improve the outcomes of endodontic treatment.

Root canal morphology of the mandibular second premolar: a systematic review and meta-analysis

Background The aim of this paper was to systematically review the root canal configuration (RCC) and morphology literature of the mandibular second premolar (Mn2P). Methods Systematic research of five electronic databases was performed to identify published literature concerning the root canal configuration (RCC) of the Mn2P up through July 2020. Studies were selected according to predefined search terms and keywords inclusion criteria: “root canal configuration”, “root canal system”, “root canal morphology”, “mandibular second premolar”, “mandibular premolars”, “morphology” and “anatomy”. Further possible studies were identified by cross-referencing and screening the bibliographies of the selected articles. Results From 1622 retrieved studies, 44 studies investigating the internal morphology of 17,839 Mn2Ps were included. Most examined Mn2Ps were single-rooted (89.5–100%); two-rooted (0.1–8%) and three-rooted (0.1–3.5%) Mn2Ps at lower frequency. Most frequent RCCs reported were 1–1–1/1 (55.3–99.6%) followed by 1–1–2/2 (0.5–57%) and 2–2–2/2 (0.6–18%). The meta-analysis of seven studies demonstrated that a significantly higher number of RCC type 1–2–1/1 (OR [95%CI] = 2.05 [1.27, 3.33]) and 2–2–2/2 (OR [95%CI] = 2.32 [0.65, 8.63]) were observed in male than in female patients. Conclusions Different RCC research methods have been reported. Whereas clearing and radiographs were commonly used in the past, CBCT has been prevalent in recent years. A globally high frequency of a 1–1–1/1 RCC in the Mn2P has been reported. Nevertheless, the probability that different, more complicated RCCs can appear in Mn2Ps should not be underestimated and, thus, should be taken into consideration when making decisions during an endodontic treatment.

Objetivo: Determinar la anatomía interna y externa de los primeros premolares inferiores mediante tomografía computarizada de haz cónico, diafanización y estereomicroscopio. Materiales y Métodos: Estudio observacional, descriptivo con muestra no probabilística por conveniencia de 300 primeros premolares mandibulares en base a un registro de 150 tomografíascomputarizadas de haz cónico (TCHC) con campo de visión 11x8y 90 órganos dentales extraídos. Se analizaron las (TCHC) para determinar la presencia del conducto en forma de C y observar el tipo de conductos radiculares. Con respecto a los 90 primeros premolares inferiores selos dividió en 3 grupos para evidenciar la presencia de hendiduras radiculares, bifurcación radicular y determinar el número y tipo de conductos radiculares de acuerdo a la clasificación de Vertucci. Resultados: En relación a las 150 (TCHC), se pudo evidenciar la presencia de conducto en forma de C un 14.7%. Con respecto a la disposición de conductos según Vertucci se determinó un solo conducto que corresponde al tipo I con el 86,7%, seguido por el tipo V con el 6,0%, tipo III con el 5,7 %, tipo VI con el 1,3 % y el tipo VII con el 0,3%. De los 90 órganos dentalesse identificó con Estereomicroscópio que un 33,33% presentó la hendidura radicular y en un 3,33% la bifurcación radicular, mediante (TCHC) se determinó una mayor presencia de un conducto único y con la técnica de Diafanización unamayor frecuencia del Tipo I de Vertucci con un 36,7%. Conclusión: Los primeros premolares mandibulares constituyen un verdadero reto para el endodoncista debido a su amplia variación morfológica interna y externa
