Objective: The purpose of this investigation was to assess clinician and patient feedback about voice therapy using a variably occluded face mask (VOFM), and to determine if voice therapy augmented via a VOFM would result in favorable changes in patient self-perceived handicap, as well as acoustic and aerodynamic measures. Methods/Design: Pilot, prospective, pre-post single group design. Eleven patients with dysphonia due to primary muscle tension dysphonia (8) or benign vocal fold lesions (3) were recruited. Data collected included patient and clinician feedback of voice therapy using a VOFM, voice handicap index (VHI)-10, acoustic and aerodynamic measures. Data were collected before treatment (baseline) and one-week post therapy. Wilcoxon signed-rank tests were used to compare data pre and post therapy. Results: Statistically significant improvement was observed for the VHI-10 with a median delta of -7. Clinician feedback generally reported that patients like the VOFM, using the VOFM within the first two sessions of therapy, and within less than 10 minutes of use. All clinicians ranked the conversation level of the hierarchy as the most effective level. Three themes emerged from the Therapy Feedback Form: the SOFM was a 1) “Facilitator for Sensation,” 2) a “Physical Tool”; and that there was 3) “No Program Needed” to use the VOFM in voice therapy. There was a statistically significant improvement in CPP (p=0.0329) and CSID (p=0.0164) in sustained vowels. Discussion: This pilot study represents the first investigation into clinician and patient perceptions of using a variably occluded face mask (VOFM). Reported measures via patient perception, as well as clinician perceptions, and some acoustic and aerodynamic measures showed that participants got better with VOFM voice therapy. Last, in general, both clinicians and patients liked utilizing a VOFM in voice therapy.

Rehabilitation of a voice function in patients with benign laryngeal lesions after surgical treatment

Usually benign laryngeal lesions like cysts, polyps and fibrous masses occurs in result of phonotrauma as reactive changes of the mucous of vocal cords. Presence of organic pathology of larynx is a reason of functional voice disorders. 45 patients in age of 18-70 y.o. with benign laryngeal lesions were investigated. All patients underwent microlaryngoscopy with excision of the lesion in inpatient mode. This article describes complex of pre- and postoperative procedures performed to assess the vocal function in patients. For the surgical excision of lesions «cold-steel» instruments, radiofrequently ablation and laser surgery were used. Authors analysed the correlation between complaints, acoustic voice parameters and the process of mucous recovery. Laser technique is recommended for surgical treatment of benign lesions of larynx. Fibrolaryngoscopy, laryngostroboscopy and computer acoustic assessment of vocal parameters are required before voice normalization. Phonotherapy is indicated n case of persistence of dysphonia.

Introducción. Los docentes son usuarios ocupacionales de la voz con alto riesgo de desarrollar patologías vocales a causa de su labor. Para enfrentar esta problemática, suelen usarse ejercicios con tracto vocal semiocluido (TVSO), estrategia de educación/rehabilitación empleada para generar un cambio en el patrón de vibración de los pliegues vocales, minimizando el riesgo de lesión vocal al reducir el estrés de colisión al que se someten los tejidos. Diversos reportes han indicado que este tratamiento tiene efectos en el cociente de cierre (CQ), medida indirecta del estrés de colisión. Objetivo. Examinar el efecto fisiológico de dos ejercicios con TVSO en la actividad laríngea de profesores con esfuerzo vocal constante pero sin patología vocal. Metodología. Se registraron muestras de 43 profesores antes, durante y después de la realización de dos ejercicios con TVSO (vibración lingual y fonación en tubos). Las muestras de electroglotografía se analizaron para obtener el CQ. Resultados y conclusión. Se observó una diferencia significativa al comparar los valores del CQ antes y durante la realización de ambos ejercicios. No se encontraron cambios en este parámetro después de ejecutar la actividad. Estos hallazgos concuerdan con reportes previos en los cuales el CQ tiende a aumentar durante la fonación en tubos; contrariamente, la realización de la vibración lingual tiende a decrecer el valor del CQ. Es posible que este comportamiento se deba a los mecanismos biomecánicos particulares de cada ejercicio.
