Investigation of the Cardiac Effects of Pancuronium, Rocuronium, Vecuronium, and Mivacurium on the Isolated Rat Atrium

Abstract: The heart rate effect of pancuronium and developed force effects of pancuronium, vecuronium, rocuronium, and mivacurium may occur via direct stimulation of β receptors. Although our investigation was an in vitro study, the effects found may be important especially under pathologic conditions, such as hypertension, in which patients usually use β-blocking agents, which cause β receptor upregulation.

“…These findings suggested that cardiac effects of these agents were induced at high concentrations. Similarly, in a study by Gursoy et al., 7 cardiac effects of non-depolarizing NMBDs were examined in isolated rat atrium and the results were consistent with the relevant literature. 8 This previous study suggested that such effects could be particularly important in hypertensive patients on β-blockers in clinical practice.…”

“…These effects may be attributed to direct stimulation of β-adrenoceptors or pathological conditions, such as hypertension, where β-blocking agents, are generally used. 5 , 7 , 8 Cardiovascular effects of NMBDs on the isolated atrium have been studied on rats. 8 Because endothelial cells play a significant role in regulating hemostasis, 9 cardiovascular effects could be mediated by the endothelium.…”

“…Two factors are probably responsible for this finding. First, we paralyzed the rats with pancuronium, an agent which increased baseline heart rate by its vagolytic action (Marshall 1973;Gursoy et al 2011). Our use of pancuronium might have limited stimulation-induced increases in cardiac output, which in turn might have contributed to an increase in arterial pressure.…”

“…Other neuromuscular blockers like pancuronium and atracurium were used in the past to control the muscle spasm. However, their use was not recommended as Pancuronium causes tachycardia [15] and atracurium such as bradycardia and hypotension [16] which may trigger mortality in the patient with tetanus. Based on the above evidence, Vecuronium is now recommended to be used as a neuromuscular blocker in Tetanus.…”
